| Objective:All cases of uterine fibroids selected in this clinical study met the inclusion criteria.A total of 90 cases were selected.To observe the clinical efficacy of HIFU combined with Guizhi Fuling pills in the treatment of uterine fibroids,and to seek an efficient and safe treatment for patients with uterine fibroids.Methods:A total of 90 cases of uterine fibroids hospitalized in Maternal and Child Health Hospital of Jiangxi Province from January 2020 to August 2020 were selected.Premenopausal women older than 18 years.Meet the inclusion criteria,Preoperative TCM syndrome differentiation all belong to the blood stasis syndrome type.Before treatment,the relevant doctor should explain the treatment method,expected effect,precautions and possible complications in detail to the patient and the patient’s family.All patients signed the relevant informed consent before surgery.All the selected patients were randomly divided into Guizhi Fuling pill combined with HIFU group and HIFU group.HIFU combined with Guizhi Fuling pill was used as the observation group,and HIFU group was used as the control group,with 45 cases in each group.The observation group was treated with HIFU first,and Guizhi Fuling Pill was taken orally for 3 months after operation,and stopped taking during menstrual period.The control group was simply treated with HIFU,and no drugs for uterine fibroids were taken after surgery.The symptoms and signs of the two groups of patients before surgery were basically the same,and the general information was not statistically significant,but comparable.All the included patients came to our hospital for reexamination 1 month and 3 months after the operation.The reexamination items were mainly B-ultrasound and hemoglobin(HGB),and the patients’ symptoms and changes in quality of life were asked.For multiple uterine fibroids,the diameters of the largest fibroids were measured.Observation and comparison of the two groups of patients in the postoperative uterine fibroid reduction rate,menstrual changes,quality of life and other aspects of the impact.Results:1.Uterine fibroid volume:(1)intra-group comparison: there was no significant difference in the uterine fibroid volume 1 month after surgery between the two groups and the preoperative comparison(P> 0.05);Compared with preoperative,both groups were reduced 3 months after surgery,and the difference was statistically significant(P<0.05).(2)Comparison between groups: Comparison of fibroid volume 3 months after surgery between the two groups,the reduction of the observation group was better than the control group(P<0.05);2.Reduction rate of uterine fibroids: a comparison was made between groups 1 month after surgery and 3 months after surgery,and the reduction rate of uterine fibroids in the observation group was better than that in the control group,with statistical significance(P<0.05).3.Improvement of TCM syndromes:(1)The total score of TCM syndromes was improved in both groups 1 month and 3 months after surgery(P<0.05),and further improved in both groups 3 months after surgery compared with1 month after surgery(P<0.05),but the improvement in the observation group 3 months after surgery was better than that in the control group(P<0.05).(2)One month after operation,the single syndrome scores of lower abdominal mass and dysmenorrhea in the observation group were improved,while those of the control group were not significantly changed.Three months after operation,the syndrome scores of lower abdominal mass, menstrual volume,lower abdominal pain,menstrual color and quality,prolonged menstrual period and dysmenorrhea in the observation group were all improved,while the syndrome scores of lower abdominal mass and dysmenorrhea in the control group were only improved,but the improvement degree of each single symptom score in the observation group was better than that in the control group.4.Comparison of hemoglobin(HGB)changes:(1)intra-group comparison:HGB values of both groups were improved 3 months after surgery,and the difference was statistically significant(P<0.05).(2)Intergroup comparison: HGB values of the two groups 3 months after surgery were compared,and the improvement of HGB in the observation group was better than that in the control group,the difference was statistically significant(P<0.05).5.Uterine fibroid symptoms and health-related quality of life(UFS-QOL)score:(1)intra-group comparison: UFS score at 1 month after surgery decreased in both groups compared with that before surgery,the difference was statistically significant(P<0.05);The UFS score at 3months after surgery was further decreased in both groups compared with1 month after surgery(P<0.05).Compared with preoperative QOL scores,QOL scores of both groups were improved 1 month after surgery,and the difference was statistically significant(P<0.05).QOL scores in both groups were further increased 3 months after surgery compared with 1 month after surgery(P<0.05).(2)Comparison between groups: Comparison of UFS scores in the two groups at 1 month and 3 months after surgery showed that the decrease degree of UFS score in the observation group was better than that in the control group,the difference was statistically significant(P<0.05).Comparison of QOL scores in the two groups at 1 month and 3 months after operation showed that the improvement degree of QOL scores in the observation group was better than that in the control group,with statistical significance(P<0.05).6.Comparison of clinical efficacy: in the observation group 3 months after operation,16 cases were significantly effective(35.56%),27 cases were effective(60%),and 2 cases were ineffective(4.44%),with a total effective rate of 95.56%;In the control group,3 cases were significantly effective(6.67%),35 cases were effective(77.78%),7 cases were ineffective(15.56%)and the total effective rate was 84.44% at the 3rd month after operation.Chi-square test confirmed that the clinical efficacy of the observation group was better than that of the control group(P<0.05).Conclusion:Both groups showed significant clinical efficacy.After clinical observation and comparative analysis,it was found that HIFU combined with Guizhi Fuling pill(observation group)in the treatment of uterine fibroids had a higher reduction rate of fibroids than the control group,with a faster reduction in fibroids volume,a faster increase in hemoglobin,a more significant improvement in UFS-QOL score,a more significant improvement in symptoms,and a significantly improved quality of life.In conclusion,Guizhi Fuling pill combined with HIFU in the treatment of uterine fibroids is a better choice,worthy of promotion and application. |
