| 1 Objective:Based on thermal aggregates lung phlegm elderly multiple drug resistant pseudomonas aeruginosa(MDR-PA)application of traditional Chinese medicine compound pneumonia patients blood poisonous clear(the first affiliated hospital of anhui medical university da-yong zhou experience),director of the clinical trials,each adapting just-in-time inventory index before and after treatment were observed(%,PCT,CRP,WBC,can only learn,IL-6)changes,as well as the clinical comprehensive efficacy,efficacy of bacteriology,the skills he-65 score,SOFA score and APACHE II,the change of TCM syndrome integral,etc,The intervention effect of Xueduqing on the infection index,etiological efficacy and clinical treatment effect of the elderly patients with MDR-PA pneumonia with phlegm-heat accumulation of lung was further investigated,and its clinical value was evaluated to provide the basis for clinical syndrome differentiation and treatment.2 Methods:Collected on June 1,2019 to January 31,2021 the first affiliated hospital of anhui medical university general intensive care unit(GICU)included in the standard of 46 cases diagnosed senile multiple drug resistant pseudomonas aeruginosa pneumonia(lung phlegm heat yun)patients cases,using the function calculator for simple randomized groups,46 cases were randomly divided into control group and treatment group.This study mainly on the branch of "the Chinese medical association respiratory disease diagnosis and treatment of community-acquired pneumonia guide(2016edition)","hospital acquired pneumonia and ventilator associated pneumonia diagnosis and treatment guidelines(draft)(2018 edition)","2014 years of diagnosis and treatment of lower respiratory tract in pseudomonas aeruginosa infection experts consensus" and the main points of syndromes of traditional Chinese medicine and instructions for diagnosis and treatment of related standards.Matched group shall be positive anti-infection,phlegm cough,respiratory,nutritional support,such as conventional western medicine therapy,treatment group on the basis of control group therapy combined with traditional Chinese medicine medicinal broth blood poisonous clear by the first affiliated hospital of anhui medical university(tisanes chamber unified generation Fried),nasogastric or oral drugs,a bag,a bag of 100 ml,once in the morning and night.Take 14 days as a course of treatment.The two groups were treated for 14 days consecutively,and the infection indexes(WBC,NEUT%,PCT,CRP,IL-6,HBP),bacteriological efficacy,clinical comprehensive efficacy,Curb-65 score,SOFA score,APACHE II score,Chinese medicine symptom complex score and other trends were compared between the two groups before and 14 days after treatment.3 Results:(1)The levels of infectious indexes(WBC,NE%,PCT,CRP,IL-6,HBP)in 2 groups were decreased after treatment(P < 0.05),and the decrease in the treatment group was more obvious than that in the control group after 14 days of treatment(P < 0.05).(2)The comprehensive clinical efficacy of the two groups.After 14 days of treatment,the total effective rate of the comprehensive curative effect in the treatment group was higher than that in the control group,and the difference between the two groups was statistically significant(P < 0.05).(control group 52.17%,treatment group 82.60%).(3)The bacteriological efficacy of patients in the two groups.The bacterial clearance rate in the treatment group was higher than that in the control group,and the difference between the two groups was statistically significant(P<0.05).(control group 26.08%,treatment group 56.52%).(4)SOFA score,Curb-65 score,Apache II score and TCM syndrome score were significantly decreased after treatment compared with before treatment(P < 0.05),and the decrease was more significant in the treatment group(P < 0.05).4 Conclusion:(1)The traditional Chinese medicine compound Xueduqing can effectively improve the levels of WBC,NE%,PCT,CRP,IL-6,HBP and other related infectious indexes in the elderly patients with multi-drug resistance Pseudomonas aeruginosa pneumonia of phutm-heat accumulation lung type,indicating that Xueduqing has a positive effect on the control of inflammatory response in the elderly patients with multi-drug resistance Pseudomonas aeruginosa pneumonia.(2)Compared with the simple use of western medicine control group,the traditional Chinese medicine compound blood poisonous clear combined western medicine can improve the pseudomonas aeruginosa bacteria clearance,related studies have shown that blood poisonous clear in traditional Chinese medicine(TCM)in addition to the simple anti-inflammatory antimicrobial also can improve the body’s immune function,explain TCM intervention from,and the traditional Chinese medicine bacteriostasis of healthy qi,the improve pathogen clearance,to improve the curative effect of pathogeny,alleviate the pressure of the use of antimicrobial drugs,and so on.(3)Chinese medicine compound thermal aggregates lung blood poisonous clear can reduce phlegm elderly multiple drug resistant pseudomonas aeruginosa pneumonia patients SOFA scores,the skills he-65 score,APACHE II score and TCM syndrome integral comprehensive score,showed that blood poisonous clear can effectively improve thermal aggregates lung phlegm in elderly patients with MDR-PA overall situation,help relieve clinical symptoms,delaying disease progression and improve the prognosis of patients with outcome. |
