Effects Of The Heat-sensitive Moxibustion “Dubi” Acupoint On P-P38MARK,P53 And Caspase3 In Rabbit With Knee Osteoarthritis

Objective:To observe the effects of thermosensitive moxibustion at "Dubi" on P-P38 mitogen activated protein kinase,tumor suppressor gen e P53 and Caspase-3 levels of dorsal root ganglia,articular cartila ge in knee osteoarthritis rabbit model,and to explore the mechanism of thermosensitive moxibustion in KOA based on the direction of cart ilage apoptosis.Methods: Thirty 6-month-old New Zealand healthy white rabbits with a mass of 2.25~2.75 kg were randomly divided into a blank group(6),a model group(6),and a moxibustion group(18).After 3 days of adaptive feeding,the blank group was fed normally with free drinking without any intervention;The remaining groups were modeled for 15 days.The specific method was as follows: Inject 0.5ml of 4% papain solution into the articular cavity of the right posterior knee of the rabbit,once every 3 days,for a total of 3 injections,and force the rabbits to move for 1 hour every day.After modeling,the blank group did not have any intervention,and the rabbits in the model group were fixed in a rack for 40 minutes a day for a total of 7 days of intervention;The moxibustion group received gentle suspension moxibustion on the right "dubi" for 40 minutes,once a day for 7 days,During the treatment,the thermal tomography imaging system and temperature recorder were used to measure the temperature of the rabbit’s abdomen,hip joint,ankle joint and other areas.According to the temperature fluctuation of 7 days,the heat-sensitive moxibustion group and the non-heatsensitive moxibustion group were divided.10 mice with continuous temperature fluctuation>1℃ were classified as heat-sensitive moxibustion group;7 mice with continuous temperature fluctuation ≤1℃ were classified as non-heat-sensitive moxibustion Moxibustion group,Six animals were randomly selected from each group.During the experiment,the general condition and behavioral changes of each group of rabbits were recorded.After the treatment,the morphological changes of chondrocytes were observed by light microscope and Mankin score was performed.Western blot was used to detect the expression level of P-P38 MARK protein in the spinal dorsal root ganglia,and the expression level of cartilage P53 and Caspase-3 genes were detected by Real-time PCR.Result:1.Before modeling,there were no obvious abnormalities in the mental state,diet and other general conditions of the rabbits in each group.After modeling,the rabbits in the blank group were in general condition,and the Lequesne MG score was 0.Model rabbits showed poor energy,significantly reduced diet and drinking,curled up in the corners,exercised less,limp when forced to exercise,and even jumped on one leg;Lequesne MG score was significantly increased(P<0.01).2.After treatment,the general condition of the rabbits in the blank group and the Lequesne MG score did not change significantly.The general condition of the rabbits in the model group,non-heatsensitive moxibustion group,and heat-sensitive moxibustion group improved,and the Lequesne MG score decreased,but it was still significantly higher than the blank group.(All P<0.01).rabbits in the model group recovered more slowly,and the non-heat-sensitive moxibustion group was less improved than the heat-sensitive moxibustion group;in terms of Lequesne MG score,compared with the model group,the non-heat-sensitive moxibustion group and the heat-sensitive moxibustion group The Lequesne MG scores of rabbits all decreased(P<0.05,P<0.01),but there was no significant difference between the groups(P>0.05).3.After treatment,the Mankin score,the expression levels of p53,caspase-3 in articular cartilage and p-p38 mark in dorsal root nerve in the model group were significantly higher than those in the blank group(P < 0.01);compared with the model group,the Mankin score,the expression levels of p53,caspase-3 in articular cartilage and pp38 mark in dorsal root nerve in the non heat sensitive moxibustion group and the heat sensitive moxibustion group were decreased(P <0.05,P < 0.01),and the heat sensitive moxibustion group was better than the non heat sensitive moxibustion group Heat sensitive moxibustion group(P < 0.05).Conclusion:1.Heat-sensitive moxibustion can improve the general condition and behavioral performance of the KOA rabbit model,indicating the effectiveness of heat-sensitive moxibustion in the treatment of KOA.2.Heat-sensitive moxibustion can significantly reduce the Cartilage Mankin score,dorsal root nerve P-P38 MARK,articular cartilage P53 and Caspase-3 levels in the KOA rabbit model,regulate the apoptosis of chondrocytes,and repair cartilage damage.This is a possible effect of heat-sensitive moxibustion in the treatment of KOA.mechanism.
