| Objective: This study summarized the distribution and disease characteristics of TCM syndromes in elderly myeloid malignancies in Jiangxi Province,in order to better guiding clinical diagnosis and treatment.Methods: The medical history data of 120 elderly patients with myeloid malignancies living in Jiangxi Province who met the inclusion criteria were collected.The four diagnosis data of all patients were reviewed by physicians with the title of deputy chief TCM physician or above,and a medical record data database was established.SPPS26.0 software was used for statistical analysis of the patients’ residence,age,gender,common symptoms,tongue pattern,pulse pattern,syndrome type,peripheral blood pattern,bone marrow hyperplasia,previous complicated diseases,course of disease,Chinese medicine medication,etc.Results:(1)The 120 patients in this study were all from different cities in Jiangxi Province,including 71 males(59.17%)and 49 females(40.83%).The incidence rate of males was higher than that of females,and the average age was 70.36±6.80 years old.(2)Among the 120 patients,there were 46 cases of spleen deficiency and phlegm-dampness syndrome,26 cases of qi and blood deficiency,20 cases of qi and Yin deficiency syndrome,and 20 cases of stasis and poison combination syndrome.The proportion of spleen deficiency and phlegm-dampness syndrome was the highest(38.33%).(3)There was statistical significance between TCM syndrome type and WBC count and platelet count(P<0.05),but no statistical significance between TCM syndrome type and hemoglobin count(P>0.05).The proportion of white blood cells lower than normal was the largest,among which,the proportion of deficiency syndrome of Qi and blood was the largest(80.77%),and the proportion of high white blood cells was the lowest.The proportion of platelet lower than normal value was more than half,among which the proportion of Qi and Yin deficiency syndrome was the largest(95.83%).The proportion of moderate anemia in the syndrome of spleen deficiency and phlegm dampness,the syndrome of Qi and Yin deficiency,and the syndrome of stasis and toxin combination all exceeded half,among which the proportion of severe anemia in the syndrome of Qi and blood deficiency was the largest(46.15%),which can provide reference for clinical syndrome differentiation and treatment.(4)There was no statistically significant difference between TCM syndromes and myeloid hyperplasia(P>0.05).The myeloid hyperplasia was most active or significantly active,which was in line with the characteristics of the disease.(5)120 cases of patients with clinical common symptoms of frequency from high to low in turn for dizziness fatigue,less food,stay,bleeding,dry mouth,mouth pain,body weight,fever,sweat heart hot shortness of breath,palpitation,insomnia,hands and feet,aches,dizziness fatigue,less food,stay,bleeding the proportion is more,in line with the clinical manifestations of the disease.(6)Among the 120 patients,a large number of patients with previous combined diseases were heart disease,hypertension,diabetes,cerebrovascular disease and pulmonary disease.There was no statistical significance in the relationship between TCM syndromes and complicated diseases(P>0.05).The syndrome of deficiency of qi and blood accounted for the largest proportion in heart disease(44.68%),and the syndrome of deficiency of qi and blood accounted for a large proportion in hypertension(38.46%)and diabetes(38.46%).Due to the relatively small number of cases,the sample size can be further expanded for further clinical studies.(7)The relationship between TCM syndrome type and disease course was statistically significant(P<0.001).Among the 120 patients,82 had a disease course of > of 6 months,and 38 had a disease course of less than6 months.Among the patients with a disease course of > of 6 months,spleen deficiency and phlegm dampness were the most common(40 cases,accounting for 48.78%,accounting for 33.33% of the total number of cases).The number of cases with stasis in the course of disease ≤6 months was the most,21 cases(52.26%),accounting for 87.5% of the total.In Jiangxi Province,the elderly patients with myeloid malignant hematologic tumors with short course of disease were mostly acute onset and rapid transmission,which were seen in stasis and toxin syndrome,and the elderly patients were mostly seen in spleen deficiency and phlegm dampness syndrome.(8)Among the 120 patients,116 patients(96.67%)took TCM decoction,4 patients did not take TCM decoction,116 patients used 204 TCM decoction in total,According to the frequency of its occurrence,the top 20 were Atractylodes Atractylodes(58.62%),Astragalus membranaceus(51.72%),Angelica angelica(44.83),Tuckahoe(41.38),Dangshen(34.48%),Pinellia ternata(32.76%),Paeonia lactiflora(31.90%),Bupleurum bupleurum(31.03%)and Herba sinensis(30.17%),respectively radix pseudostellariae(26.72%),dried tangerine or orange peel(25.86%),main licorice root(24.14%),yam(22.41%),platycodon grandiflorum(19.83%),madder(18.97%),cohosh(17.24%),semen coicis(17.24%),Fried hovenia dulcis(16.38%),swollen section(14.66%),caulis spatholobi,wind,12.93%).The traditional Chinese medicine used mainly has the effect of replenishing qi and blood,invigorating spleen and dampness,regulating qi and resolving phlegm.Conclusion:(1)The incidence of elderly myeloid malignancies in men was higher than that in women,with an average age of 70.36±6.80 years.(2)The most common TCM syndromes were spleen deficiency and phlegm dampness,followed by deficiency of qi and blood,deficiency of qi and Yin,and stasis and poison.(3)Common clinical manifestations in the top six are dizziness,fatigue,lack of food and stay,bleeding symptoms,dry mouth bitter,body emaciation,fever.(4)In the past,the cases of complicated diseases were mainly heart disease,hypertension,diabetes mellitus,cerebrovascular disease and pulmonary disease.(5)The lower white blood cell value was mainly seen in the syndrome of Qi and blood deficiency,while the higher white blood cell value was mainly seen in the syndrome of stasis and poison.Severe anemia was common in the syndrome of Qi and blood deficiency.Platelet lower than normal value is commonly seen in Qi and Yin deficiency syndrome.(6)The longer course of disease is more common in the syndrome of spleen deficiency and phlegm dampness,while the shorter course of disease is more common in the syndrome of stasis and poison.(7)The traditional Chinese medicine used mainly has the effect of replenishing qi and blood,invigorating spleen and dampness,regulating qi and resolving phlegm.The understanding of TCM syndromes distribution and disease characteristics in this study can provide reference for TCM syndrome differentiation and diagnosis and treatment of elderly myeloid malignant hematologic tumors in Jiangxi Province. |
