Colorectal cancer(CRC),including colon cancer and rectal cancer,is a common digestive system malignant tumor in human being.It ranks second in the gastrointestinal cancer.Its incidence rate is showing an increasing trend.Experts predict that the incidence rate of and CRC will soon exceed that of gastric cancer,and become the main cause of death in gastrointestinal cancer.With the rapid development of China’s economy,the Westernization of diet structure,habits and habits,and the changing of living environment,the incidence rate of CRC in China is also increasing.Early detection,early diagnosis and early treatment of CRC can significantly improve the five-year survival rate.The incidence of CRC varies in different regions and races of the world,and there are significant differences in the prevalence of CRC in different regions of China.With the improvement of economic level,medical and health level,residents’ health awareness,changes in living and eating habits,and the advancement of population aging in Baoding,the prevalence of CRC in Baoding will also change accordingly.This paper analyzes,compares and summarizes the prevalence of CRC in different age groups in Baoding,and understands its internal laws and characteristics,so as to provide reference for the development of CRC in this region To provide more theoretical and reference basis for cancer diagnosis and treatment.Objective:Objective to provide reference for the diagnosis and treatment of CRC by comparing the clinical characteristics of patients with CRC at different ages in Baoding.Methods:1705 cases of CRC patients diagnosed in our hospital(Baoding first central hospital)from January 2015 to December 2019 were selected as the research objects.They were divided into young group(< 45 years old),middle-aged group(45-59 years old)and elderly group(≥ 60 years old)according to their age)To analyze the changes of the number of patients with CRC with age,and the changes of the number of patients in the three groups with years.To compare the general information(gender composition,education level,body mass index,smoking history,drinking history,family history),clinical data(location of disease,first symptom,polyp,distant metastasis at diagnosis),pathological characteristics(pathological type,adenocarcinoma differentiation degree,gross type,TNM stage),blood examination(the changes of hemoglobin,CEA and CA19-9 at the time of diagnosis)of the three groups were compared The changes of CEA and CA19-9 at diagnosis were different.Linear correlation analysis was used to analyze the correlation between the two variables;Chi square test or Fisher exact probability method were used to compare the three samples,P <0.05 was defined as statistically significant difference.Bonferroni method was used for pairwise comparison among multiple samples.P < 0.0167 was defined as statistically significant difference.Results:1.With the growth of age,the number of patients with CRC gradually increased,and the number of patients with CRC was positively correlated with age(P<0.05).2.With the growth of years,the number of patients with CRC in young,middle-aged and elderly groups showed an increasing trend,and the number of patients was positively correlated with the year(P<0.05).3.General information comparison3.1 There was no significant difference between male and female in different age groups(P>0.05).3.2 The proportion of primary school education in the elderly group was higher than that in the young and middle-aged groups(P<0.05),and the proportion of junior high school education in the young and middle-aged groups was higher than that in the elderly group(P<0.05).3.3 The proportion of normal weight in the elderly group was higher than that in the young and middle-aged groups(P<0.05),and the proportion of overweight in the young and middle-aged groups was higher than that in the old group(P<0.05).3.4 There was no significant difference in smoking history among different age groups(P>0.05).3.5 There was no significant difference in drinking history among different age groups(P>0.05).3.6 The proportion of the young group with family history was higher than that of the old group(P<0.05).4.Comparison of clinical data4.1 The incidence of right colon cancer in young group was higher than that in middle-aged and old group(P<0.05),and the incidence of rectal cancer was lower than that in middle-aged and old group(P<0.05).4.2 Abdominal pain was the most common symptom in the young group(P<0.05),and the proportion of abdominal distension was also higher than that in the elderly group(P<0.05).In the elderly group,hematochezia or occult blood in stool were the most common(P<0.05).4.3 The incidence of polyps in elderly patients was higher than that in middle-aged group(P<0.05),and that in middle-aged group was higher than that in young group(P<0.05).4.4 There was no significant difference in distant metastasis among different age groups(P>0.05).5.Comparison of pathological features5.1 The proportion of mucinous adenocarcinoma in young group was higher than that in middle-aged and old group(P<0.05)5.2 The proportion of poorly differentiated adenocarcinoma in young and middle-aged group was higher than that in old group(P<0.05).5.3 The proportion of mass type in young and middle-aged patients was higher than that in old patients(P < 0.05),and the proportion of infiltrating type in old patients was higher than that in young and middle-aged patients(P<0.05).5.4 In terms of TNM staging,the proportion of stage I and II patients in the elderly group was higher than that in the young and middle-aged groups(P<0.05),and the proportion of stage IV patients in the young and middle-aged groups was higher than that in the elderly group(P<0.05).6.Comparison of blood test results6.1 At the time of diagnosis,the proportion of decreased hemoglobin in the elderly group was higher than that in the middle-aged group(P <0.05).6.2 There was no significant difference in CEA among different age groups(P>0.05).6.3 There was no significant difference in the change of CA19-9among different age groups(P>0.05).Conclusion:With the increase of age,the number of CRC patients increased gradually.The number of young,middle-aged and elderly CRC patients in Baoding area increased year by year.The education level,body mass index,family history,location of disease,first symptoms,combined polyps,pathological type,differentiation degree of adenocarcinoma,gross type,TNM stage and hemoglobin at diagnosis of CRC patients in different age groups were significantly different There are differences.The education level of CRC patients in this region is not high,and the pathological stage of patients is late.Therefore,it is necessary to strengthen the publicity and education of CRC knowledge in this region,so as to make people realize the harm of smoking and drinking,and the body mass index of people in this region tends to increase.We should develop good eating and living habits to avoid overweight and obesity.In the young group,the symptoms of CRC are concealed,the pathological types are poor,the differentiation degree is low,the clinical stage is late when diagnosis is made,and the malignant degree is high.Therefore,we should improve the understanding of young people suffering from CRC,and adopt comprehensive detection methods,so as to achieve the purpose of early diagnosis and treatment and improve the prognosis.
