| Objective:In the context of building a healthy China,in order to further implement the strategy of strengthening the grassroots level,this paper analyzes the difficulties and problems in the allocation of health costs in Shanxi Province in recent years,analyzes the effect of health services provided by primary medical and health institutions,and studies the opportunities and challenges in the development of primary medical and health institutions,so as to provide the basis for optimizing health policy measures and enhancing the development momentum of primary medical and health institutions.Method:Consult the "China Health Statistics Yearbook" and other related materials,collect relevant data on the allocation and utilization of resources of primary medical and health institutions in Shanxi Province,use description and comparison methods,conduct research on the collected data,and analyze the problems existing in primary medical and health institutions in our province.Through multi-stage random sampling,the community of 5 counties/districts in the province was selected for questionnaire distribution,and indicators such as composition ratio and χ2 test were used to study the relationship between the improvement of circulatory system diseases and the utilization of primary health services.Result:1.The total amount of health expenditure in Shanxi Province increased and the health expenditure accounted for more than 6% of GDP,and the total health expenditure per capita was 2715.91 yuan,which was lower than the national average.The total health expenditure in Shanxi Province is on the rise as a whole,but health expenditure is not compatible with the level of economic development.2.Under the method of financing source,the proportion of government health expenditure is 31.05%,which is higher than the national average.The average proportion of social health expenditure in Shanxi Province was 36.40%,but the overall growth rate was slow.The proportion of personal cash expenditure in Shanxi Province is 32.55 %,but it is still much higher than the national average.3.Hospitals account for the largest proportion of health expenditures in Shanxi Province,and primary medical and health institutions account for only 6.33%.The health investment of township health centers is higher than that of community health service centers,and there are differences between urban and rural health investment.4.Circulatory system disease is the disease with the most serious disease burden.It accounts for a quarter of all disease burdens in the province.The main sources of financing for circulatory system disease are government health expenditures and personal cash expenditures.The main treatment sites are concentrated in hospitals.5.From 2014 to 2018,the average annual growth rate of primary health institutions in Shanxi Province was 0.86%.The number of beds and the total number of health technicians increased steadily,and the number of family doctors increased by95.92 million times.However,there was a gap in the number of registered nurses and general practitioners,and there were differences in the utilization of urban and rural primary health institutions.6.The circulatory system diseases with the most serious disease burden were selected as the research object.It was found that primary medical and health services have obvious relevance to the improvement of circulatory system diseases.Improving the service capabilities of family doctors can promote the function of primary medical and health institutions.Conclusion:1.The development of primary health care institutions in Shanxi Province is good,but there is still a gap with the goal of " strong grassroots."2.The structure of health financing in Shanxi Province needs to be optimized.We s hould redesign the payment method of social medical insurance,and promote the design of head payment scheme for general practitioner contracted services.3.The rational use of primary health care services can effectively improve the health of residents and reduce the burden of individual diseases. |
