Objective:Objective To explore the prognostic factors of gram-positive cocci bloodstream infection in intensive care unit(ICU).Methods:A retrospective analysis of 130 patients with gram-positive cocci bloodstream infection in the ICU of the First Affiliated Hospital of Anhui Medical University from January2013 to June 2019.Patients were divided into survival group 72 cases and death group58 based on death within 28 days For example,the physiological indicators and laboratory parameters of the two groups of patients were compared,and the relevant risk factors affecting the 28-day prognosis of patients with gram-positive cocci bloodstream infection were analyzed..Results:112 cases(86.15%)of 130 patients with gram-positive cocci bloodstream infection were staphylococcal infections,including 38 cases(33.23%)of Staphylococcus epidermidis,25 cases of hemolytic Staphylococcus 19.23%,and 17 cases of Staphylococcus human(13.08%),12 cases of Staphylococcus aureus(9.23%).There were 18 cases(13.85%)of Enterococcus.The results of univariate analysis showed that the survival group and the death group were in hypertension,septic shock,APACHE II score,SOFA score,methicillin-resistant staphylococcus,not receiving appropriate empiric antibiotic treatment,prealbumin,creatinine,blood urea Nitrogen(BUN),arterial blood lactic acid and other indicators were significantly different(P<0.05).Multivariate logistic regression analysis showed that the total number of days in hospital(OR=0.967),septic shock(OR=11.580),methicillin-resistant Staphylococcus infection(OR=16.864),and not receiving appropriate empiric antibiotic treatment(OR=6.533)),APACHEII score(OR=1.160),SOFA score(OR=1.449)are independent risk factors that affect the 28-day prognosis of ICU gram-positive cocci bloodstream infectionConclusions:Septic shock,methicillin-resistant staphylococcal infection,failure to receive appropriate empiric antibiotic treatment,APACHEII score,SOFA score are independent risk factors for the prognosis of ICU gram-positive cocci bloodstream infection.
