Objective:1.WBC count and classification,HsCRP,PCT and IgE concentrations in peripheral blood of children with non-asthmatic pneumonia group,asthmatic predictive index(API)negative asthmatic pneumonia group and API positive asthmatic pneumonia group were observed to understand the differences in the levels of above indicators among the three groups.2.Serum concentrations of YKL-40 MCP-1 and Eotaxin in children in the above groups were determined to study the differences among different groups and the correlation between various indicators to explore the role of YKL-40 MCP-1 and Eotaxin in the pathogenesis of pneumonia and asthmatic diseases.Methods:1.Pneumonia cases hospitalized in the Department of Pediatrics,Affiliated Hospital of Guilin Medical College from October 2019 to December 2020 were collected and divided into non-wheezing pneumonia group(group A)and API-negative asthmatic pneumonia group(group B)and API-positive asthmatic pneumonia group(group C)according to their clinical characteristics.2.The age and gender of the children were recorded,their body length and weight were measured,and HsCRP,PCT and IgE in peripheral blood were detected by routine blood tests.3.The concentrations of YKL-40 MCP-1 and Eotaxin in peripheral blood were determined by ELISA.4.SPSS 23.0 software was used for data processing and analysis.Measurement data was expressed as median(quad interval)[M(P25,P75)].Kruskal-Wallis test was used for comparison between three groups,and Mann-Whitney test was used for non-parametric test U Pairwise comparison was performed between the three groups,and Spearman correlation analysis was used to analyze the correlation between the two variables.P<0.05 was considered statistically significant.Results :1.A total of 320 pneumonia cases meeting the inclusion criteria were collected,including 155 cases in group A and 150 cases in group B and 15 cases in group C.2.General Information:The ratio of male to female in the three groups was 82/73,110/40,10/5,and the weight [M(P25,P75)] was 8.5(6.5,11.0)kg,9.0(7.5,10.0)kg,11.0(9.6,12.0)kg,respectively.There were significant differences in gender and age among the three groups(X2=13.754,P=0.001;H = 8.850,P = 0.012).3.WBC count and classification :(1)WBC count of group A [M(P25,P75)] was 9.24(6.87,12.45)*109/L,group B was 10.62(7.37,12.44)*109/L,group C was 12.13(7.70,14.29)*109/L,the difference between the three groups was not statistically significant(H=2.457,P=0.293);(2)The absolute value of lymphocytes [M(P25,P75)] in group A,B and C was 4.87(3.60,6.37)*109/L,5.45(3.77,7.65)*109/L,5.07(1.90,6.98)*109/L respectively,the difference between the three groups was not statistically significant(H=3.887,P=0.143);Lymphocyte ratio [M(P25,P75)] in group A,B,C were 57.9(40.0,70.9)%,56.3(42.8,69.7)%,47.8(29.8,62.4)%,no statistically significant difference among three groups(H = 4.319,P = 0.115);(3)The absolute value of neutrophils [M(P25,P75)] in group A,B and C was 2.38(1.38,5.83)*109/L,3.19(1.70,4.66)*109/L,4.46(2.28,7.36)*109/L,the difference between the three groups was not statistically significant(H=5.220,P=0.074);Neutrophils ratio [M(P25,P75)] in group A,B,C were 27.7(17.5,49.0)%,31.7(18.2,44.1)%,35.6(27.6,59.6)%,no statistically significant difference among three groups(H = 4.994,P = 0.082);(4)The absolute value of eosinophils [M(P25,P75)] in group A,B and C was 0.11(0.04,0.27)*109/L,0.09(0.02,0.21)*109/L,0.24(0.02,0.69)*109/L respectively.The difference between the three groups was not statistically significant (H=4.184,P=0.123);Eosinophil ratio [M(P25,P75)] in group A,B,C were 1.1(0.3,3.2)%,0.9(0.3,2.3)%,2.3(0.5,4.9)%,no statistically significant difference among three groups(H = 4.668,P = 0.097).4.Peripheral blood HsCRP,PCT and IgE test results:(1)The concentration of peripheral blood hs CRP [M(P25,P75)] in group A,B and C was 2.51(0.44,20.60)mg/L,and 1.49(0.32,6.79)mg/L,2.48(0.98,10.76)mg/L respectively,the difference between the three groups was not statistically significant(H=5.493,P=0.064);(2)The concentration of peripheral blood PCT [M(P25,P75)] in group A,B and C was 0.10(0.07,0.20)ng/ml,0.08(0.06,0.14)ng/ml,0.06(0.05,0.11)ng/ml respectively,the difference between the three groups was statistically significant(H=10.628,P=0.005);(3)The peripheral blood IgE concentration [M(P25,P75)] in group A,B and C was 76.0(13.7,251.4)ng/ml,111.3(32.9,310.5)ng/ml,648.9(242.2,1518.0)ng/ml respectively,the difference between the three groups was statistically significant(H=16.766,P<0.001).5.The results of serum YKL-40 MCP-1 and Eotaxin were as follows :(1)Serum YKL-40 concentrations [M(P25,P75)] in group A,B and C were 24.19(19.87,34.96)ng/ml,17.75(11.50,32.81)ng/ml,17.38(13.45,30.93)ng/ml respectively,the difference between the three groups was not statistically significant(H=3.011,P=0.222);(2)Serum MCP-1 concentration [M(P25,P75)] in group A,B and C was 288.72(248.83,430.52)pg/ml and 411.70(287.78,689.63)pg/ml,319.42(278.98,419.38)pg/ml respectively,the difference between the three groups was not statistically significant(H=3.785,P=0.151);(3)Serum Eotaxin concentrations [M(P25,P75)] in group A,B and C were 64.83(53.56,80.27)pg/ml,64.12(46.56,90.33)pg/ml,63.42(45.78,90.47)pg/ml respectively,the difference between the three groups was not statistically significant(H=0.027,P=0.987).6.Correlation between white blood cell count,neutrophil ratio,HsCRP and PCT:(1)WBC was positively correlated with neutrophil ratio and HsCRP(r=0.243,P<0.001;r= 0.230,P<0.001),had no significant correlation with PCT(r=0.004,P=0.945);(2)The neutrophil ratio was positively correlated with HsCRP and PCT(r=0.553,P<0.001;r= 0.259,P<0.001);(3)There was a positive correlation between HsCRP and PCT(r=0.541,P<0.001).7.Correlation analysis between YKL-40 MCP-1 and Eotaxin:(1)Eotaxin was positively correlated with MCP-1(r=0.318,P=0.013);(2)There was no significant correlation between YKL-40 and MCP-1(r=0.147,P=0.258);(3)There was no significant correlation between YKL-40 and Eotaxin(r=-0.021,P=0.874).8.Correlation analysis of YKL-40,MCP-1 and Eotaxin with white blood cell count and classification,HsCRP,PCT and IgE:(1)YKL-40 was positively correlated with the absolute value of neutrophils,HsCRP and PCT(r=0.276,P=0.031;r= 0.419,P=0.002;r=0.421,P=0.002),there was no significant correlation between YKL-40 and other indicators(P>0.05);(2)There was a positive correlation between MCP-1 and lymphocyte ratio(r=0.253,P=0.049),and a negative correlation between MCP-1 and absolute neutrophil value(r=-0.284,P=0.027),but no significant correlation between MCP-1 and other indexes(P>0.05);(3)There was a negative correlation between Eotaxin and PCT(r=-0.290,P=0.035),but no significant correlation between Eotaxin and other indicators(P>0.05).Conclusions :1.People with elevated serum IgE are more likely to have wheezing symptoms in infant pneumonia.2.Overweight,male may be a predisposing factor for wheezing in infants and young children.3.The wheezing manifestations of infants with pneumonia are mostly caused by infection,so the serum MCP-1 and Eotaxin levels are not significantly different from those of children without wheezing pneumonia.4.Serum YKL-40 concentration is not correlated with MCP-1 and Eotaxin levels,which does not support the use of serum YKL-40 to predict the future wheezing process of infant patients.
