Background: With the development of social economy and the progress of medical and health technology,the life expectancy of China’s population has been greatly extended,and the aging trend of the whole society is becoming more and more obvious.Like the aging of all other organs,the natural aging of the skin and the changes of skin function with age make the elderly more likely to suffer from various dermatosis.This not only reduces the quality of life of the elderly,increases the economic burden of individuals and families,but also some dermatoses(such as severe drug eruption,skin tumors)can also seriously affect the quality of life of the elderly.Therefore,to explore the prevalence of senile dermatosis and its related factors,strengthen the prevention and treatment of the disease,and effectively improve the life and quality of life of the elderly has become one of the hot issues in dermatology.Objective: Retrospectively analyzed the prevalence,disease composition and related factors of elderly inpatients with dermatosis,and the clinical diagnosis and treatment of elderly severe drug eruption in Anhui,so as to accumulate relevant clinical data for clarifying the occurrence regularity of elderly dermatosis and provide scientific basis for the prevention and treatment of severe drug eruption.Methods: 3051 elderly patients aged 60 or older were admitted to the Department of Dermatology,the first affiliated Hospital of Anhui Medical University from January2014 to December 2019.The disease types,disease composition,clinical characteristics and drug inducement and other related data in the elderly were systematically analyzed retrospectively.The classification of diseases referred to the diagnostic criteria of Chinese Clinical Dermatology edited by Zhao Bian.The data were statistically analyzed by SPSS20.0.The statistical method was ?2 test of paired comparison of enumeration data and the discrepancy was considered of statistical significance when p<0.05.Results:(1)3051 elderly inpatients accounted for 28.53% of the total number of dermatology inpatients in the same period,and the ratio of male to female was 1.36:1.There are 18 categories and 57 kinds of dermatosis,among which the top three are allergic dermatosis 908 cases(29.76%),viral infectious dermatosis 880 cases(28.84%),and immune bullous disease 406 cases(13.31%).Stratified analysis of disease categories shows that the top six dermatoses that led to hospitalization in the elderly were herpes zoster 880 cases(28.84%),eczema 396 cases(12.98%),drug eruption 280cases(9.18%),bullous pemphigus 278 cases(9.11%),psoriasis 225 cases(7.37%)and urticaria 145 cases(4.75%).The sex comparison of disease types showed that there were more male patients with eczema and psoriasis than female patients(male: female = 4.08:1.0 and 2.88 Suzhou 1.0),while herpes zoster,drug eruption and urticaria were more common in female patients(male: female = 0.93,0.96 and 0.38).About 74%(2258)of the 3,051 patients were complicated with one or more chronic diseases,and hypertension(39.43%),chronic respiratory diseases(18.42%)and diabetes(15.80%)were the first three.(2)Among the 50 elderly patients with severe drug eruption,there were 21 cases of SJS(42%),17 cases of TEN(34%),11 cases of ED(22%)and 1 case of DHS(2%),,including 22 males and 28 females.The average latency of onset was 17.63 ±12.15 days.Antiepileptic drugs ranked first(11 cases,22%),and allopurinol(10 cases,20%)was the most common type of allergenic drugs.43 cases were cured or improved(the total effective rate was 86%),7 cases failed in treatment,of which 1 case died.Conclusion:(1)Elderly inpatients with dermatosis account for nearly 1/3 of the average annual total number of inpatients,among which allergic dermatosis ranked first,and herpes zoster is the most hospitalized dermatosis.(2)SJS and TEN were the most common types of severe drug eruptions in the elderly.Antiepileptic drugs and allopurinol were the most common allergenic drugs.The total effective rate of treatment is 86%.
