pregnancy,great changes have taken place in their physiology and psychology,and their sleep patterns have also changed significantly,which has seriously affected their sleep quality.Poor sleep quality not only seriously interferes with the quality of life and mental health of pregnant women,but also has a negative impact the growth and development of the fetus.Orexin-A is an important awakening peptide and plays an key role in regulating the rhythm of sleep-wake cycle.Lack or over-activation of Orexin-A can lead to unstable sleep-waking states,causing sleep quality problems.Previous studies on Orexin-A mainly focused on animal mechanisms and cellular level studies,and clinical studies need to be supplemented.Moreover,few studies have focused on pregnant women.The role of Orexin-A in the sleep quality of pregnant women is unclear.Therefore,it is of great significance to study the influencing factors of sleep quality and its relationship with orexin-A in pregnant women,so as to prevent the occurrence of sleep problems and promote the healthy development of mother and fetus.ObjectivesThis study was to investigate the prevalence of sleep quality in pregnant women and their influencing factors,and to explore the correlation between serum Orexin-A and sleep quality in pregnant women.In order to provide ideas for the research on the mechanism of sleep quality during pregnancy,and provides a scientific theoretical basis for clinical nurses to evaluate the sleep quality of pregnant women and make effective intervention.Methods1.Using convenience sampling method,519 pregnant women who met the inclusion criteria were selected from two general hospitals in Xinxiang city as the research objects.The participants were surveyed with general information questionnaire,Pittsburgh sleep quality index,self-rating depression scale,self-rating anxiety scale,and pregnancy stress scale.Descriptive analysis,Chi-square test,u test,Mann-Whitney U test,Spearman correlation analysis,Pearson correlation analysis and binary logistic regression analysis were performed using SPSS24.0 software.2.214 pregnant women who volunteered to participate in the experiment and took serum samples were selected from the cross-sectional study,and their serum orexin-A levels were detected by enzyme-linked immunosorbent assay(ELISA).In addition to the above statistical analysis methods,another Kruskal-Wallis H test Bonferroni test was used for statistical analysis.Results1.The score of PSQI was(5.80±3.09)and the incidence of sleep disorders in the pregnant woman was 59.73%.2.There were significant differences in incidence of sleep disorders among pregnant woman with different gestation stage,education levels,employment status,pre-pregnancy sleep quality evaluation and exercise frequency during pregnancy(P<0.05).In addition,the differences in incidence of sleep disorders among pregnant woman with different depression scores,anxiety scores and pregnancy stress scores were statistically significant(P<0.001).3.The results of regression analysis showed that pregnant women in the first trimesters(OR=0.509,95% CI:0.302~0.857)and second trimesters(OR=0.474,95% CI:0.286~0.785)had lower risk of sleep disorders than those in the third trimester;pregnant women with low pre-pregnancy sleep quality evaluation(OR=2.381,95% CI:1.784~3.177)and high anxiety scores(OR=1.042,95% CI:1.003~1.082)had a higher risk of sleep disorders.4.Serum Orexin-A level was [0.29(0.21)] ng/ml in pregnant women.The serum Orexin-A level of the non-sleep disorder group was [0.27(0.11)] ng/ml,and the serum Orexin-A level of the sleep disorder group was [0.33(0.34)] ng/ml.5.The difference of serum Orexin-A level among different sleep quality groups in the first,second and third trimester of pregnancy was statistically significant(P<0.05).There was statistical significance in the serum Orexin-A level of pregnant women with satisfactory sleep evaluation before pregnancy in different sleep quality groups(P<0.05).The difference of serum Orexin-A level in different sleep quality groups was statistically significant in non-anxiety pregnant women(P<0.001).6.The difference of serum Orexin-A levels in different gestation stages was statistically significant(P<0.001).Further pairwise comparisons showed that the Orexin-A levels of first trimester was significantly different from that of second trimester and third trimester,respectively(P<0.05).Conclusions1.Sleep quality in pregnant women is generally poor;gestation stage,pre-pregnancy sleep evaluation,and anxiety scores are important factors affecting sleep quality of pregnant women.2.It suggests that the serum Orexin-A levels is closely related to the sleep quality of pregnant women.The higher the level of serum Orexin-A,the poorer the level of sleep quality;serum Orexin-A levels were higher in the second trimester and third trimester than in the first
