BackgroundThe incidence rate of stroke in young adults is increasing year by year,and the proportion of ischemic stroke is higher,which makes the patients’ families and society suffer from extremely severe emotional trauma and heavy economic burden.Many previous researches have shown that atherosclerosis is the main cause of young patients with ischemic stroke.At present,there are few studies on the relationship between the degree of neurological deficit and carotid plaque in young patients with ischemic stroke.In this study,taking the young patients with ischemic stroke as the research object,with the assistance of ultrasound evaluation of patients with carotid plaque,according to the National Institute of Health Stroke Scale(NIHSS)score,to explore the relationship between the degree of neurological deficit and carotid plaque,and analyze the risk factors of young patients with ischemic stroke,in order to find a new idea for the early prevention and treatment of young patients with ischemic stroke.ObjectiveTo investigate the relationship between the degree of neurological deficit and carotid plaque in young patients with ischemic stroke by in-depth analysis of the risk factors of young patients with ischemic stroke,which is capable of providing the basis for early prevention and treatment of high-risk young individuals with ischemic stroke and disease assessment of the patients.Methods1.According to the inclusion criteria,109 young patients with ischemic stroke admitted to the Department of Neurology of The First Affiliated Hospital of Xinxiang Medical College from July 2018 to October 2019 were selected as the case group,and 71 healthy young adults aged 18~45 in The First Affiliated Hospital of Xinxiang Medical College physical examination center for physical examination during the same period were selected as the control group,the general data including the results of biochemical indexes were recorded.2.Ultrasound examination of cervical vessels was taken in both groups,carotid Intima Media Thickness(CIMT),plaque thickness and degree of carotid stenosis were detected and recorded at the common carotid artery and its bifurcation and 2cm from the beginning of internal carotid artery,the number of plaques,crouse score and carotid atherosclerosis grade score of the two groups were counted,the average of the three measurement results were taken.3.According to the NIHSS score,the patients in the study group were divided into mild stroke group(NIHSS ≤ 3,68 cases)and non-mild stroke group(NIHSS > 3,41 cases).The relationship between the degree of neurological deficit and carotid plaque index was analyzed.Results1.The number of plaques,carotid atherosclerosis grade score and crouse score of young patients with ischemic stroke were higher than those of the control group.2.The number of plaque,carotid atherosclerosis grade score and crouse score in non-mild stroke group were higher than those in mild stroke group.3.Logistic regression analysis showed that TG,LDL,HCY,BMI,carotid atherosclerosis grade score and crouse score were risk factors of young patients with ischemic stroke.4.The ROC curve was constructed and the area,sensitivity and specificity under the curve were compared,indicating that the carotid atherosclerosis grade score and crouse score have certain predictive value for the incidence of young patients with ischemic stroke.5.while the grade score of carotid atherosclerosis and crouse score were positively correlated with NIHSS score of young patients with ischemic stroke.Conclusions1.TG,LDL,HCY,BMI,carotid atherosclerosis grade score and crouse score were risk factors of young patients with ischemic stroke.2.Carotid atherosclerosis grade score and crouse score have certain predictive value for the onset of young patients with ischemic stroke.3.Carotid atherosclerosis grade score and crouse score were positively correlated with the degree of neurological deficit of young patients with ischemic stroke. |
