In 2009,the Central Committee of the Communist Party of China and the State Council released the opinions on deepening the reform of the medical and health care system to the public,and the new medical reform clearly stated that general medical treatment should be guided to the grass-roots level.Our country insurance basic,strong basic health policy requirements of basic-level medical institutions should give priority to in order to provide the treatment of common diseases,but this should be at the grassroots medical institutions to solve the rural residents of common disease and frequently-occurring disease,at present the treatment choice and flow still is in a state of concentration in higher medical institutions,this phenomenon is a waste of the limited health resources at the grass-roots level,It also increases the medical expenses of rural residents,and at the same time aggravates the pressure of superior medical institutions.The purpose of this article is to understand the current situation of common chronic medical treatment behavior of rural residents in Henan Province.,explore the related factors of improvement are put forward based on influencing factors of initial common chronic medical advice at the grass-roots level of rural residents,so as to make full use of the basic medical institutions of limited health resources,alleviate the pressure of the superior clinic medical institutions,And provide theoretical support for relevant management departments to formulate policies.Based on the above research purposes,firstly,this paper selected 500 rural patients with common chronic diseases who met the inclusion criteria in Xinxiang City,Henan Province to conduct a questionnaire survey.The research tools include:(1)he basic information table of the research objects,including gender,age,per capita monthly income,occupation,education level,etc.;(2)Questionnaire on grass-roots medical behavior,including the first medical institution,medical consideration factors and physical examination frequency,etc.;(3)Satisfaction Questionnaire of primary medical institutions,which includes the satisfaction degree of inspection equipment,drugs and medical expense reimbursement in primary medical institutions.Secondly,semi-structured interviews were conducted for rural patients with common chronic diseases who met the inclusion criteria and primary medical care managers,so as to deeply explore the factors affecting the grass-roots medical treatment behavior of rural patients with common chronic diseases and put forward suggestions.Finally,SPSS 25.0 was used to process the survey data.Questionnaire survey and interview method were used to carry out the survey based on the research objects meeting the inclusion criteria and the results were summarized into five aspects.(1)181 cases(37.3%)of rural patients with common chronic diseases were first diagnosed;304 patients(62.7%)were first diagnosed by superior medical institutions.68 cases(14.0%)were familiar with the "hierarchical diagnosis and treatment policy",and91 cases(18.8%)were familiar with the "basic reimbursement rate for chronic diseases".(2)There was a statistically significant difference between patients with chronic diseases who were willing to seek medical treatment at the grass-roots level and those who were not willing to receive medical treatment at the grass-roots level during the rehabilitation period,and patients with chronic diseases had a lower willingness to seek medical treatment at the grass-roots level.(3)Regression analysis showed that age,occupation,education level and per capita monthly income were the main influencing factors for the grass-roots medical treatment behavior of rural residents with common chronic diseases.(4)The influence factors of the hierarchical diagnosis and treatment system: the eigenvalues of occupation,age,education level and family per capita income were compared,and the differences were statistically significant.Influencing factors of reimbursement rate for chronic diseases at grassroots level: comparison of eigenvalues of three variables,gender,occupation and health status,showed statistically significant differences.Through the analysis of the interview content,the themes proposed by rural patients with chronic diseases are as follows: lack of medical facilities and low level of medical treatment in primary medical institutions;Themes put forward by primary medical care managers: rural patients with common chronic diseases have poor health awareness,low attention to chronic diseases,lack of personnel in primary medical institutions,and insufficient support from national policies.Based on the above analysis conclusions,the primary influencing factors are age,occupation,education level,per capita monthly income,the awareness of hierarchical diagnosis and treatment system,and the awareness of the reimbursement rate of chronic diseases at the grassroots level.The comprehensive service ability satisfaction score of primary medical institutions is low.In the end,this paper puts forward some suggestions from four aspects.In terms of improving the overall health cognition of rural residents,the suggestions are as follows:changing the old concept of medical treatment,strengthening the education of health knowledge and strengthening the publicity of chronic diseases;Suggestions for strengthening basic medical institutions’ infrastructure construction and improving comprehensive service ability are as follows: attaching importance to the training of basic medical personnel,perfecting the construction of basic medical equipment and optimizing the drug guarantee mechanism;In terms of increasing the publicity of the hierarchical diagnosis and treatment system and strengthening the awareness of policies,the following suggestions are proposed: pay attention to the coverage rate of the hierarchical diagnosis and treatment,expand the publicity mode of the hierarchical diagnosis and treatment and increase the publicity of the hierarchical diagnosis and treatment;In the aspect of improving the management and operation system of chronic diseases,the suggestions are as follows: to pay more attention to the prevention and control of chronic diseases,increase funding for chronic diseases and establish information platform for chronic diseases,so as to further promote the prevention and control work.
