| Objective:To explore the importance,methods and management strategies of the protection of peritumoral drainage veins in the central gyrus,so as to reduce the tumor recurrence rate and improve the quality of life of patients.Methods:The clinical data of 41 patients with meningioma in the central gyrus region treated in the Department of Neurosurgery of the affiliated Hospital of Qinghai University from December 2013 to December 2020 were analyzed retrospectively.Results:Tumor resection according to Simpson grade: grade Ⅰ + gradeⅡ resection in 38 cases(92.7%),grade IV resection in 3 cases(7.3%).There were 2patients with postoperative muscle strength of grade Ⅰ,followed up for 1 year and1.5 years,respectively,the muscle strength of the other patients returned to normal,and 2 cases of new epilepsy were cured with drugs.According to WHO pathological grade,36 cases were WHO Ⅰ and 5 cases were WHO Ⅱ.During the follow-up period from 6 months to 4 years,there were 2 cases of recurrence,the former received surgical treatment again at the 13 th month and the 20 th month after operation,and the latter was treated with gamma knife and no tumor progression was followed up for 2 years.Conclusion:It is of great significance to fully evaluate the relationship between tumor and cerebral cortex,tumor blood supply artery and drainage vein before operation and choose appropriate surgical approach for guiding microsurgical treatment of meningioma.Protecting drainage vein around tumor during operation can reduce recurrence rate of meningioma and improve patients’ quality of life. |
