Objectives:(1)To investigate the relationship between the concentration of common clinical serum antioxidants(uric acid,albumin,bilirubin)and the occurrence and rupture of intracranial aneurysms.(2)To Summarize the clinical characteristics of patients with intracranial aneurysms and to analyze the related factors that affect the rupture of intracranial aneurysms.Methods:262 patients with intracranial aneurysms in the Department of Neurology of Qinghai Provincial People’s Hospital from January 2017 to January2020 were retrospectively collected,which included 120 males(45.80%)and 142females(54.20%),with an average age of(55.99 ± 12.66)years old ranging from 20 to 84 years old.According to whether aneurysms were ruptured or not,they were divided into unruptured group(n=161)and ruptured group(n=101).The rupture group was divided into a mild group(n=70)and a severe group(n=31)according to the severity of the patient’s condition.Compare the serum uric acid,albumin and bilirubin concentrations between the unruptured group and the ruptured group.Statistically describe the clinical data(gender,ethnicity,smoking history,drinking history,history of hypertension,stroke and diabetes history)and imaging data(number,size,location,morphology of aneurysms)and compare them whether there is a difference between the unruptured group and the ruptured group.Multivariate logistic regression analysis was performed on all the different indicators.Results :(1)The concentration of uric acid in patients with intracranial aneurysms in the unruptured group was higher than that in the ruptured group,and the concentration of total bilirubin and direct bilirubin was lower than that in the ruptured group(P<0.05).(2)The biochemical indexes of 262 patients with intracranial aneurysms were tested by single sample t-test.The results showed that the serum uric acid and albumin concentrations of patients with intracranial aneurysms were lower than those of healthy control group,and the bilirubin concentration was higher than that of healthy control group,the difference was statistically significant(P< 0.05).(3)There were statistically significant differences in serum uric acid and direct bilirubin concentrations among the unruptured group,mild group,and severe group(P<0.05).Pairwise comparison showed that the serum uric acid concentration of the unruptured group was higher than that of the mild group and the severe group,and the direct bilirubin concentration of the mild group was higher than that of the unruptured group.(4)Logistic regression analysis of factors affecting intracranial aneurysms rupture showed direct bilirubin(P=0.019,OR=1.541),aneurysm morphology(P=0.010,OR=7.959),and aneurysm size(P=0.013,OR=2.086)is an independent risk factor for intracranial aneurysms rupture.Uric acid(P<0.001,OR=0.991),age(P=0.001,OR=0.955),and history of stroke(P<0.001,OR=0.215)are the protective factors for intracranial aneurysms rupture.Conclusions:(1)A higher serum uric acid concentration within the normal range may be beneficial to patients with intracranial aneurysms.The higher the concentration of direct bilirubin,the higher the risk of intracranial aneurysms rupture.(2)The decrease of antioxidant capacity(uric acid and albumin)in patients with intracranial aneurysms suggests that the increased level of oxidative stress may be involved in the pathogenesis of intracranial aneurysms.(3)The shape and size of intracranial aneurysms are independent risk factors affecting the rupture of the aneurysm.The risk of rupture of non-saccular aneurysms increased significantly.The larger the intracranial aneurysms,the greater the risk of rupture.
