| Background:To evaluate the diagnostic value of MRI Bravo sequence combined with Cube Flair sequence enhanced scan in patients with meningeal metastasis of lung cancer,so as to provide clinicians with more effective examination methods in the diagnose of meningeal metastasis of lung cancer.Methods:We retrospective studied 135 patients diagnosed lung cancer by pathology and clinically suspected leptomeningeal metastasis in our hospital from january 2019 to jaunary 2020.There were 70 patients had undergone MRI Bravo sequence combined with Cube Flair sequence enhanced scan and Cerebrospinal fluid examination.We took the results of the cerebrospinal fluid cytology as the gold standard,the diagnostic value of the MRI Bravo sequence combined with Cube Flair sequence enhanced scan in leptomeningeal metastasis of lung cancer was evaluated by calculating area under curve of the receiver operating characteristic curve,sensitivity,specificity.Results:Among the 70 patients who suspected leptomeningeal metastasis,25 patients were diagnosed by Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together,11 patients were only diagnosed by Cerebrospinal Fluid and 5 patients were only diagnosed by MRI Bravo sequence combined with Cube Flair sequence enhanced scan;41 patients were diagnosed by Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan;29 patients were negative in Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together.The result were that The AUC of Cerebrospinal fluid and Bravo combined Cube Flair MR enhancement scanning sequence in diagnosis of leptomeningeal metastasis of lung cancer was 0.774(95%CI 0.660-0.887,p<0.001),the sensitivity,specificity was69.4%,85.3% respectively.In this study,in the elevated cerebrospinal fluid protein subgroub,22 patients were diagnosed by Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together,1 patients were only diagnosed by Cerebrospinal Fluid and 2 patients were only diagnosed by MRI Bravo sequence combined with Cube Flair sequence enhanced scan,4 patients were negative in Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together;in the non-elevated cerebrospinal fluid protein subgroub,3patients were diagnosed by Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together,10 patients were only diagnosed by Cerebrospinal Fluid and 3 patients were only diagnosed by MRI Bravo sequence combined with Cube Flair sequence enhanced scan,25 patients were negative in Cerebrospinal fluid and MRI Bravo sequence combined with Cube Flair sequence enhanced scan together.The result of elevated cerebrospinal fluid protein subgroub were that The AUC of Cerebrospinal fluid and Bravo combined Cube Flair MR enhancement scanning sequence in diagnosis of leptomeningeal metastasis of lung cancer is 0.812(95%CI 0.573-1.000,p<0.05),the sensitivity,specificity,was95.7%,66.7% respectively;The result of non-elevated cerebrospinal fluid protein subgroub were that The AUC of Cerebrospinal fluid and Bravo combined Cube Flair MR enhancement scanning sequence in diagnosis of leptomeningeal metastasis of lung cancer is 0.562(95%CI 0.366-0.758,p>0.05),the sensitivity,specificity,was76.9%,89.3% respectively.Conclusions:1.MRI Bravo sequence combined with Cube Flair sequence enhanced scan is of great importantence in diagnosis of leptomeningeal metastasis of lung cancer.So,MRI Bravo sequence combined with Cube Flair sequence enhanced scan combined with cerebrospinal fluid is helpful to the diagnosis of leptomeningeal metastasis of lung cancer.2.As to MRI Bravo sequence combined with Cube Flair sequence enhanced scan,the lung cancer patients whose cerebrospinal fluid protein was elevate and clinically suspected leptomeningeal metastasis are the dominant population.Perfecting MRI Bravo sequence combined with Cube Flair sequence enhanced scan for this tepe of patients was expected to further increase the detection rate of the patients with meningeal metastasis of lung cancer. |
