Objective:The purpose of this study was to evaluate the tumor markers’ significance for auxiliary diagnosis of mammary cancer,the tumor markers such as:β2-MG、AFP、FER、PRL、CEA、CA125 、CA153,and to explore the best combination for parallel test of serum tumor markers.It provides strategies and recommendations for screening serum Tumor Markers in clinical and breast cancer diagnosis.Method:873 patients with mammary cancer and 476 patients with benign lesions diagnosed in a hospital in Nanchang from January 2018 to June 2018 were retrospectively analyzed and divided into breast cancer group and benign lesion group.Seven tumor markers were measured between the two groups.SPSS 20 software was used for data analysis,and Mann-Whitney U test was used to analyze whether there were statistical significance among the 7 tumor markers between the two groups,and the test level α was 0.05.And then calculate the Single test of seven tumor markers’ sensitivity,specificity,positive predictive value,negative predictive value and diagnosis incidence rate.Using parallel text to analyse changes in 7 tumor markers’ sensitivity,specificity,positive predictive value,negative predictive value and diagnosis incidence rate which tested after the parallel detection of two combinations,three combinations,four combinations,five combinations,six combinations and seven combinations.Result:1.The tests showed that the difference was statistically significant,by analyzing of the distribution of 7 tumor markers,including β 2-mg,AFP,FER,PRL,CEA,CA125 and CA153 in 1349 cases of breast cancer group and benign lesions group.Except PRL,the other six tumor markers’ level in breast cancer group were higher than in benign lesions group.2.When people using a single test,the sensitivity and negative predictive value were generally in a low level.However,the specificity and positive predictive value were in a high level.The diagnostic coincidence rate was universally not high.3.With the increasing of the number of tumor markers parallel test,the sensitivity increased from 15.58% to 39.18%,yet in totally,the sensitivity was not improved much;the specificity and positive predictive value were decreased with the increasing of the number of parallel test;but the negative predictive value changed a little.And theparallel test had a little effect on the value.The positive predictive value was increased slightly with the increasing of the number of parallel test.Conclusion:1.The levels were different between the breast cancer group and benign lesion group of 7 tumor markers.Except PRL,the levels in breast cancer group were higher than the benign lesion group’s.The 7 tumor markers have certain practical significance in clinical auxiliary diagnosis.2.The specificity and negative predictive value of single detection of breast cancer tumor markers were low,which had a little clinical significance for routine physical examination or breast cancer screening.3.The specificity and positive predictive value of single detection of breast cancer tumor markers were high.,which had great significance in clinical auxiliary diagnosis.CEA、CA153 or FER is recommended firstly.4.As for a routine physical examination or breast cancer screening,in consideration of the expense,β 2-mg + PRL + FER combined experiment were recommended.If the detecting fee was not into people’s consideration,the seven tumor markers can be detected at the same time.5.After the parallel test,the specificity and positive predictive value of tumor markers were still at a not high level.It was suggested that people should be used simultaneously with color doppler ultrasound or molybdenum target in practical. |
