Objectives:The aim of this study is to investigate the current situation of the primiparas’breastfeeding self-efficacy and breastfeeding knowledge level, analyze the effect of nursing interventions based on the solution focused approach on breastfeeding self-efficacy.This study provides a reference for the intervention program of breastfeeding self-efficacy and provides a theoretical basis for improving the situation of breastfeedingMethods:From May 2020 to December 2020,116 primiparas were selected from the obstetric outpatient department and inpatient department of a third grade class-A hospital in Nanchang City by convenience sampling method as research objects. The intervention group and the control group were divided by the method of coin toss.58primiparas in the first obstetric ward were selected as the intervention group and 58primiparas in the second obstetric ward were selected as the control group.Routine breastfeeding health education was provided in the control group, while the breastfeeding nursing interventions based on the solution focused approach were provided in the intervention group.A baseline survey was conducted before the intervention, and the efficacy of the intervention was evaluated at the 1st week,the42nd day,the 4th month and the 6th month postpartum. The breastfeeding self-efficacy,breastfeeding knowledge level and exclusive breastfeeding rate of primiparas were assessed by the Chinese version of BSES-SF,the breastfeeding knowledge questionnaire and the breastfeeding status questionnaire. Finally, the date was input by Excel 2019 and analyzed by SPSS 23.0.Results:1.Baseline survey results:55 cases and 52 cases of intervention group and control group completed data collection,a total of 107 cases. There were no significant differences in demographic date, breastfeeding self-efficacy and breastfeeding knowledge scores between the 2 groups before intervention(P>0.05).2.The effect of SFA on the breastfeeding self-efficacy:There was statistically significant difference in breastfeeding self-efficacy and its two factors at the 1st week, the 42nd day,the 4th month and the 6th month postpartum(P<0.05),the scores of the intervention group were higher than the control group.Through repeated measurement ANOVA,there was statistically significant difference in breastfeeding self-efficacy and its two factors in time factor and intervention factor(P<0.05),and there was an interaction between time factor and intervention factor. With the passage of time, the breastfeeding self-efficacy and its two factors on the 42nd postpartum day were improved compared with before, the intervention group increased significantly(P<0.05).Breastfeeding self-efficacy and its two factors of the intervention group increased at the 6thmonth postpartum compared with before, while control group decreased compared with the 42ndday postpartum. There were statistically significant differences in breastfeeding self-efficacy and its two factors in 2 group at 5 time points before and after intervention(P<0.05).3.The effect of SFA on the breastfeeding knowledge:There was statistically significant difference in breastfeeding knowledge between 2 groups at the each postpartum time(P<0.05),the scores in the intervention group were higher than those in the control group.4.The effect of SFA on the exclusive breastfeeding rate:There was statistically significant difference in the exclusive breastfeeding rate between two groups at the each postpartum time point(P<0.05),the rate of exclusive breastfeeding in the intervention group were higher than those in the control group.Conclusion:1.The prenatal breastfeeding self-efficacy and breastfeeding knowledge of primiparas are at a moderate level. The nursing interventions based on the solution focused approach provide psychological and information support for primiparas, can effectively improve the breastfeeding self-efficacy of primiparas.2.Nursing interventions based on the solution focused approach can improve the breastfeeding knowledge level of primiparas.3.Nursing interventions based on the solution focused approach can improve the rate of postpartum exclusive breastfeeding of primiparas, prolong the duration of exclusive breastfeeding and improve the situation of breastfeeding. |
