Objective:By analyzing the clinical characteristics of PBC patients and the statistical differences of prognostic indicators among different groups,To provide a new idea for the early diagnosis and treatment of PBC patients and the improvement of survival prognosis.Method:100 cases of PBC patients admitted to the First Affiliated Hospital of Nanchang University from January 2015 to October 2019 were retrospectively selected as the research objects.The general clinical data,time of onset,length of stay,laboratory examination results,imaging examination results,treatment methods and survival time of the patients were collected and analyzed.The prognostic factors were statistically judged by following up the patients’ survival,and the clinical features and risk factors affecting the survival and prognosis of the patients were summarized.Result:Laboratory analysis: A total of 100 patients with PBC were included,including78 women(78%)with an average age of(55.78±11.93)years.The mean time of onset of all patients was(752.51±1326.83)days,and the mean hospitalization time was(12.51±9.61)days.The main clinical manifestations were fatigue in 37 cases(37%),abdominal distension in 44 cases(44%),jaundice in 5 cases(5%),etc.91 patients(91%)were positive for AABA AMA-M2.ANA positive patients were 89(89%).ALP was increased in all patients,and γ-GT was increased in 93 patients(93%),with a mean value of(327.27±259.42)U/L.ALB decreased in 81 patients(81%),with a mean of(31.23±4.53)g/L.PT was prolonged in 50 patients(50%),PTA was decreased in 30 patients(30%),and INR was elevated in 34 patients(34%).Univariate analysis affecting prognosis: All PBC patients were followed up for survival,and their laboratory test indicators and prognosis were statistically analyzed.It was found that serum RBC(P<0.01),HB(P<0.05),PLT(P<0.01),TBIL(P<0.05),DBIL(P<0.05),ALB(P<0.01),HDL(P<0.01),LDL(P<0.05),PTA(P<0.01)may be related to the prognosis of patients The difference was statistically significant.K-M survival analysis: K-M univariate analysis was performed on the influencing factors that may be related to the prognosis of patients and the survival time of patients.It was found that patients with RBC<3.5×109/L,HB<105g/L,TBil ≥45umol/L,DBil ≥ 30umol/L,ALB<35g/L,HDL<1.15,PTA<70%,Child grade of liver function and survival time of different clinical manifestations had significant differences(all P<0.0 5).Cox multivariate analysis: Further statistical analysis of prognostic indicators showed that PTA<70% and liver Child-Pugh B were independent risk factors affecting the survival time of patients(both P<0.05).Conclusion:1.PBC is most common in middle-aged and elderly women,with main clinical manifestations of fatigue and abdominal distention,and some patients are accompanied by other autoimmune diseases.Positive AMA-M2,positive ANA,abnormal elevation of ALP and γ-GT are helpful for the diagnosis of PBC.Routine blood tests,ALT,AST,ALB,and blood clotting tests can help determine the severity of the disease.2.In all patients,RBC<3.5×109/L,HB<105g/L,TBil ≥ 45umol/L,DBil ≥30umol/L,ALB<35g/L,HDL<1.15,PTA<70%,liver function Child Pugh C and jaundice as the main clinical presentation had a significantly shorter survival time.3.PTA<70% and liver Child-Pugh grade were independent risk factors affecting the survival time of patients.For these patients,the treatment plan should be changed as soon as possible in order to prolong the survival of patients.
