Analysis Of Risk Factors For Post-transplant Lymphoproliferative Disorder After Pediatric Liver Transplantation

【Objective】To explore the risk factors of post-transplant lymphoproliferative disorder(PTLD).【Methods】Retrospective analysis of the clinical data of PTLD patients(PTLD group)who were treated or followed up in our hospital from January 1,2012 to January1,2019 with complete clinical data.Using a 1: 2 paired design,each child in was matched with 2 children who had undergone liver transplantation within the one year and did not develop PTLD during the follow-up period(control group).and collected basic information(gender,original Incidence,age of transplantation,weight),laboratory indicators before and after transplantation(hematology,liver and kidney function,blood glucose,coagulation function,blood lipids),transplantation-related indicators(transplantation method,graft weight,graft / weight Ratio,intraoperative blood loss and blood type),tacrolimus blood concentration after transplantation,viral infection(EBV and CMV infection before surgery,EBV and CMV first infection time after surgery),donor information(blood type and EBV and CMV infections before operation were included in the analysis.【Results】 This study included 24 cases in the PTLD group and 48 cases in the control group.There is no significant statistical difference between the analysis of patients’ basic information and intraoperative information(P> 0.05);about the postoperative laboratory indicators and virus infection,the single factor analysis results showed that there were statistical differences in infection,Tac blood concentration,postoperative PLT,AST,ALT and CHOL(P <0.05).Multivariate logistics regression analysis showed that EBV infection(OR = 27.329,95% CI: 3.012-247.846 P = 0.003) was an independent risk factor for PTLD.【Conclusion】 EBV infection is an independent risk factor for the onset of PTLD,patients with EBV infection after liver transplantation should be alert to the occurrence of PTLD.
