Objective: To utilize Xiaoyao powder combined with modified Xiaochaihu decoction in the treatment of patients with constipation-predominant IBS(liver-depression and spleen-deficiency syndrome),observe its treatment conditions in TCM symptom score, patients’ quality of life and bad mental state, and evaluate its clinical efficacy and safety. Methods: Inpatients and outpatients from Gansu Provincial Hospital of TCM were selected, and 80 patients conforming to the inclusion criteria of constipation-predominant IBS(liver-depression and spleen-deficiency syndrome)were chosen. They were randomly divided into treatment and control groups, with 40 cases in each group. The control group was treated with lactulose oral solution, and the treatment group was treated with Xiaoyao powder combined with modified Xiaochaihu decoction. After 4 weeks of treatment, the symptom score, life quality score and anxiety-depression score of patients in the two groups were objectively recorded, and then the clinical practical effects were compared between the two groups. After 6 weeks of drug withdrawal, the TCM symptom scores of patients in the two groups were recorded, and the long-term therapeutic effects were evaluated. Results: 1.Curative effects on TCM syndrome:(1)After different treatment, the effective rate of the treatment group was 89.74%,and the control group was 76.32%;the effective rate of the treatment group was higher than that of the control group, and the curative effect of the treatment group was superior to that of the control group(P<0.05).(2)The total scores of the two groups after treatment were all lower than those before treatment(P<0.05),the decline degree of the total scores of the treatment group was much larger than that of the control group, and the therapeutic effect of the treatment group was superior to that of the control group(P<0.05).(3)Individual TCM symptom scores: Through intra-group comparison of the individual symptom scores of the two groups after treatment, all individual symptoms were obviously better than those before treatment(P<0.05);through comparison between the two groups after treatment, the therapeutic effect of the treatment group was superior to that of the control group in terms of abdominal pain and abdominal distention, poor appetite, weakness, belching, chest pain, dysphoria and bitter taste(P<0.05),while the therapeutic effect of the treatment group was almost equal to that of the control group in terms of improving fecal character, frequency of defecation and defecation patterns(P>0.05).2.Quality of life: The life quality of patients in the two groups was both improved, but the total score of the life quality of the treatment group after treatment was apparently higher than that of the control group(P < 0.05),indicating that the treatment group had a better curative effect in terms of improving patients’ quality of life.3.Mental state: After treatment, the HAMA and HAMD scores both showed a downward trend in the two groups(P<0.05),and the treatment group was superior to the control group in terms of adjusting patients’ negative mental state such as anxiety and depression(P<0.05).4.Follow-up visit: After 6 weeks of drug withdrawal, the total effective rate of the treatment group was 82.05%,the control group was 65.70%,and the difference was statistically significant(P<0.05),suggesting that the treatment group had a better long-term therapeutic effect than the control group.5.Safety analysis: There were no obvious uncomfortable symptoms and no adverse reactions in the two groups during treatment. Conclusion: When treating patients with constipation-predominant IBS(liver-depression and spleen-deficiency type),Xiaoyao powder combined with modified Xiaochaihu decoction has a better therapeutic effect than lactulose oral solution in terms of improving TCM syndrome score, mental state and life quality, and it has no obvious toxic and side effects. Therefore,it can be widely used in clinic and is worthy to be further researched.
