| Objective: To analyze the epidemiological characteristics of chronic heart failure and its prevention and treatment status in China;To explore the value of TCM drug/non-drug therapy in chronic heart failure disease management,and to provide the basis for the full play of TCM in chronic disease management and the construction and implementation of integrated TCM and Western medicine management program;It lays a foundation for the construction of digital management platform for chronic heart failure in this region.Methods: Selected patients with stable CHF in outpatient and inpatient CHF from January2019 to December 2019 in the Cardiovascular Center of the Affiliated Hospital of Gansu University of Traditional Chinese Medicine as the research objects.They were randomly divided into an observation group and a control group.Both groups were treated with standardized drugs.Based on different management programs.Management during hospitalization: The control group was given regular health education and disease management,including basic knowledge of heart failure,how to monitor symptoms,drug treatment plans and dependence,diet and exercise guidance,psychological and spiritual guidance,lifestyle intervention,etc.,and supervision Patients take regular medications on time,get rid of bad habits(such as smoking,alcohol abuse,staying up late,etc.),regular outpatient follow-ups,and control of other underlying diseases and comorbidities.The observation group was given traditional Chinese medicine and non-medicinal management on the basis of routine management,including taking traditional Chinese medicine,medicated diet recommendation,special Chinese medicine exercises(Tai Chi),teaching patients simple health maintenance skills and methods,and mental conditioning.Post-discharge management: A combination of regular outpatient follow-up and telephone follow-up is adopted.Follow-up will be conducted in the3 rd,6th,9th,and 12 th months after discharge.During follow-up,follow the guidelines and Duffy’s "Standard Telephone Follow-up Manuscript" for the prevention and treatment of underlying diseases Guidance and assessment of relevant knowledge in 5 aspects,including life habits,diet,exercise,and psychology,to understand the patient’s condition changes and perform TCM syndrome scores,NYHA cardiac function classification,left ventricular ejection fraction,Minnesota quality of life(MLHFQ)Score,anxiety and depression score,etc.Results: Before and after management,TCM syndrome score,TCM syndrome score effective rate,NYHA cardiac function grade,NYHA cardiac function grade effective rate,LVEF,NT-pro BNP,MLHFQ score,anxiety,depression score and re-hospitalization times were improved in the two groups compared with those when included,with statistical significance(P < 0.05).After 12 months of follow-up,the effect of observation group was better than control group,and the difference was statistically significant(P < 0.05).However,the two groups had no statistical significance in the comparison of mortality(p>0.05).Conclusion: The implementation of management for patients with chronic heart failure can improve the quality of life of patients,reduce symptoms,relieve depression and anxiety,reduce the number of hospitalizations,improve patients’ cardiac function indicators,and the integrated management of traditional Chinese and western medicine is more obvious,patients benefit more,but the impact of short-term management on mortality is not obvious. |
