Objective:Gestational Diabetes mellitus patients with different body mass were investigated for their serum levels of Irisin and FGF19 their association with insulin resistance in GDM.Methods:Selection in January 2019 to December 2019 to the first affiliated hospital of bengbu medical college antenatal examination is 24 to 28 weeks of gestation weeks pregnant women as the research object,based on the results of oral glucose tolerance test and exclusion criteria,divided into 45 cases OGTT experiment results negative for pregnant women as normal control group(NGT)and 65 cases of OGTT experiment result positive pregnant women for GDM group.According to BMI of pre-pregnancy,the GDM group can be divided into GDM1 group(25 cases of non-overweight and obesity group,prepregnancy BMI<24)and through GDM2 group(40 cases,being overweight or obese group,pre-pregnancy BMI≥24).All subjects were asked about their pre-pregnancy weight,measured their current weight and height,and calculated their pre-pregnancy body mass index and current body mass index.Blood glucose and insulin levels at each site were detected,and general clinical biochemical indexes such as TC,TG,HDL-C and LDL-C were determined.Serum Irisin and FGF19 concentrations were determined by ELISA.SPSS 23.0 statistical software was used for statistical analysis of the data.Results:Compared with NGT,the serum Irisin level in GDM1 group and GDM2 group was lower,and the Irisin level in GDM2 group was higher than that in GDM1group[(127.44±23.81)VS(104.2±18.62)VS(133.52±21.72),P<0.05];The serum FGF19 level in the GDM2 group was lower than NGT and GDM1 group[(12.97±3.61)VS(26.58±3.43)/(22.48±4.84)P<0.05],while the serum FGF19 level in the GDM1 group was not significantly different from that in the NGT group.The correlation analysis showed that the Irisin and FGF19 levels in GDM group were negatively correlated with HOMA-IR(r=-0.598、-0.349,P 均<0.05).Multiple linear regression analysis showed that:pre-pregnancy BMI,FPG,FINS and TG were the main influencing factors of Irisin;FPG,FINS and pre-pregnancy BMI were the main influencing factors of FGF19.Logistic binary regression analysis showed that pre-pregnancy was positively correlated with HOMA-IR.Logistic binary regression analysis showed that prepregnancy was positively correlated with HOMA-IR(OR=1.949,95%CI:1.484-2.561,P<0.01);Irisin and FGF19 were negatively correlated with HOMA-IR(OR=0.976、0.949,95%CI:0.958-0.996、0.919-0.980,P 均<0.05).Conclusion:1.The serum levels of Irisin and FGF19 in GDM are lower than the normal pregnant women,which are negatively correlated with insulin resistance,and their levels may be effected by pre-pregnancy BMI,blood glucose and insulin resistance before conception;2.Irisin and FGF19 are protective factors for GDM.
