A Qualitative Study About Influencing Factors Of The Compliance Of Yam Porridge On Women With Gestational Diabetes Mellitus

Objective With the capability,opportunity,motivation-behavior(COM-B)model as the theoretical framework,through qualitative interview,we explored the influencing factors of patients with gestational diabetes mellitus on compliance of yam porridge,to provide improvement strategies for improving the clinical application of yam porridge,and to provide methods and basis for expanding the clinical application of traditional Chinese medicine dietotherapy.Methods From November 2019 to November 2020,the GDM patients who had been interviewed by yam porridge dietotherapy in the maternity ward and outpatient department of a 3A hospital were taken as the research objects,and the interview outline was drawn up under the theoretical framework of COM-B model.Semi-structured one-on-one interviews were conducted among subjects who met the inclusion criteria by purpose sampling,and subjects were stopped from being included when the interviews were saturated.The recorded materials were transcribed within 12 h after the interview and analyzed by qualitative content analysis(Marying)to identify themes and categories.Results A total of 19 subjects were included finally.The 16 respondents took yam porridge more than five times a week and had good compliance.Three respondents took yam porridge three or four times a week with medium compliance.There were five factors affecting the compliance of patients with gestational diabetes mellitus to yam porridge: 1.The influencing factors of yam porridge on health,including improvement of body condition and control of blood sugar level;2.Relevant knowledge factors of yam porridge,including the function of yam,relevant knowledge of constitution,and the way to obtain knowledge;3.Preparation factors of the yam porridge,including the proficiency of preparing the yam porridge,the compliance with the preparation method of the yam porridge,the desire for a convenient preparation method of the yam porridge and acquisition and preservation of yam;4.The eating experience of yam porridge,including good eating experience,bad eating experience,desire for a variety of flavors;5.Social identity factors of traditional Chinese medicine dietotherapy,including lack of knowledge and recognition of the significance of traditional Chinese medicine dietotherapy on health.Conclusions The factors affecting the compliance of patients with gestational diabetes mellitus to yam porridge included the influencing factors of yam porridge on health,relevant knowledge factors of yam porridge,preparation factors of the yam porridge,the eating experience of yam porridge,social identity factors of traditional Chinese medicine dietotherapy.Among them,the influencing factors of yam porridge on health and the acquisition factor of yam were contributing factors.The obstacle was the preservation factor of yam.Relevant knowledge factors of yam porridge,preparation factors of the yam porridge,the eating experience of yam porridge,social identity factors of traditional Chinese medicine dietotherapy were the contributing factors or obstacles.According to the interview results,we could improve the GDM patients’ compliance of yam porridge by strengthening diet education,providing traditional Chinese medicine dietotherapy knowledge and access to knowledge,such as professional channels,new media information platform,traditional media;Providing a convenient preparation method of the yam porridge so as to enhance compliance with the preparation method of the yam porridge;Improving the taste of the yam porridge and enriching the formula of the yam porridge according to individual requirements;Promoting the commercialization of traditional Chinese medicine dietotherapy,and integrating traditional Chinese medicine dietotherapy into daily life.
