| Objective Three-dimensional reconstruction of the enhanced CT image of the inferior right hepatic vein(IRHV)and careful observation and discussion are carried out to analyze its clinical application value.Methods The imaging data of inpatients who underwent abdominal-enhanced CT in the imaging department of Handan First Hospital from December 1,2018 to December 1,2019 were collected.Finally,278 patients met the standards and all those who met the requirements were obtained.The image is edited and observed by software,and the CT image is processed by three-dimensional reconstruction software to clearly reveal IRHV,right hepatic vein(RHV)and inferior vena cava(IVC),and measure and statistically analyze related data:IRHV incidence,IRHV tube diameter,quantity,RHV tube diameter and the correlation between the three,the angle between IRHV and IVC,the distance between IRHV and RHV,and the single-factor and multi-factor correlation analysis on the incidence of IRHV.Results The incidence of IRHV is 32.73%(91/278);IRHV pipe diameter is 2.54~8.23mm,average pipe diameter(4.26±0.89)mm;RHV pipe diameter is 3.27~15.52mm,average pipe diameter(9.53±2.47)mm;The distance between IRHV and RHV is 2.41~7.13cm,the average distance is(4.63±1.05)cm;the angle between IRHV and IVC is 42.62~98.31°,of which acute angle accounts for 12.09%(11/91),Right-angle or nearly right-angle accounted for 87.91%(80/91);IRHV pipe diameter was negatively correlated with the number of IRHV(r_s=-0.12,P=0.046);RHV pipe diameter and IRHV pipe diameter were negatively correlated(r_s=-0.17,P=0.005);the results of univariate analysis showed that the diameter of RHV(t=8.926,P<0.001)was an influencing factor of the incidence of IRHV,and the result of multivariate analysis suggested that the diameter of RHV(OR=2.106,95%CI:1.377~3.223,P=0.001)is an independent influencing factor of the incidence of IRHV,and the area under the ROC curve for the RHV diameter to predict the incidence of IRHV is 0.874.Conclusion CT three-dimensional reconstruction can clearly show the anatomical structure of the intrahepatic veins including IRHV;IRHV has a certain clinical significance in liver surgery,and provides new diagnosis and treatment ideas for liver resection,Budd-Chiari syndrome(BCS)treatment,and liver transplantation;The diameter of IRHV is negatively correlated with the number of IRHV;The diameter of RHV is negatively correlated with the diameter of IRHV;the diameter of RHV has a certain influence on the incidence of IRHV. |
