| Objective:To investigate the correlation between neutrophils count(NEUT)and the stenosis of responsible vessels,severity and prognosis of acute atherosclerotic cerebral infarction(AACI).Methods:A total of 424 patients with AACI admitted to the Department of Neurology of Wuhan Fifth Hospital from June 2017 to January 2021 were collected continuously.The general conditions and previous history of the patients were asked.Blood analysis,imaging and other auxiliary examinations were completed and recorded.Groupings: According to the method of the North American Symptomatic Carotid Endosectomy Trial Study(NASCEET),the degree of responsible vessel stenosis was divided into the mild stenosis group(209 cases)and the moderate and severe stenosis group(215 cases).The severity of the disease was assessed using the National Institutes of Health Stroke Scale(NIHSS),and the patients were divided into mild stroke group(212 cases)and non-mild stroke group(212 cases).The neurological recovery of the patients was assessed using the modified Rankin scale(MRS),and the patients were divided into good prognosis group(307 cases)and poor prognosis group(117 cases).The differences of age,gender,history of hypertension,diabetes mellitus,neutrophil count(NEUT),lymphocyte count(LYM),hypersensitive C-reactive protein(hs-CRP),low density lipoprotein(LDL-C)and other indexes in each group were analyzed.In order to explore the AACI responsible vessel stenosis,the severity of the disease and the possible influencing factors of prognosis.Results:The comparison of the general baseline characteristics and statistical analysis results of the degree of responsible vessel stenosis in AACI patients showed that NEUT,LYM and LDL-C had statistical significance(P < 0.05).The variables with P < 0.05 were included in Logistic regression,and after adjusting for the influence of various factors,it was shown that the stenosis degree of the offending vessels was correlated with NEUT and LYM(P < 0.05).The predictive effect of NEUT and LYM on the degree of vascular stenosis in patients was further discussed.ROC curve was drawn and the AUC value of the area under the curve was calculated.The results showed that NEUT(AUC=0.617,95%CI=0.564-0.670,P < 0.05)was calculated.The maximum Youden index was 0.228,the corresponding Cut-off value was 4.935×109/L,the corresponding sensitivity was0.549,and the specificity was 0.679,that is,when Neut was higher than 4.935×109/L,the risk of predicting the aggravation of the stenosis of the offending vessels was significantly increased.The comparison and statistical analysis of the general baseline characteristics between the mild stroke group and the non-mild stroke group showed that there were statistically significant differences in NEUT,LYM,and hs-CRP(P < 0.05),and the severity of the patients was not correlated with NEUT,LYM,and hs-CRP after adjusting for the influence of various factors by binary Logistic regression.In order to evaluate the prognosis of patients,statistical analysis was conducted,and NEUT,LYM and hs-CRP were statistically significant(P < 0.05).The variables were included in Logistic regression,and the results after correcting for the influence of various factors showed that there was no correlation between the prognosis of patients and NEUT,LYM and hs-CRP.Conclusions:Neut can predict the severity of responsible vessel stenosis in AACI patients to a certain extent,and can be used as an effective indicator for early assessment of the severity of responsible vessel stenosis. |
