Objective:This study observed and compared the clinical efficacy of two different release depths of acupotomy in the treatment of neck type cervical spondylosis,in order to provide more clinical reference value for the release depth of acupotomy in the treatment of neck type cervical spondylosis. Methods:A total of 56 patients with cervical spondylosis diagnosed and treated by the second Department of Orthopaedics and Traumatology, affiliated people’s Hospital of Fujian University of traditional Chinese Medicine from November 2019 to December 2020 were selected. The patients were divided into experimental group(n = 28)and control group(n =28).The experimental group used acupotomy deep acupuncture release, needle depth of30 mm to 40 mm,take points: bilateral Tianzhu acupoint,Dazhui acupoint, bilateral cervical Bailao acupoint, bilateral Tianzhu acupoint. The control group was treated with acupotomy shallow needling release, and the needle depth 10 mm treatment point was the same as that of the experimental group. Both groups were treated once a week for 3 times. The VAS score before treatment,1 week after the first treatment and 1 week after the third treatment, the NDI index before and 1 week after the third treatment, and the SR value of the posterior cervical muscle before and 1 week after the third treatment were recorded. The overall efficacy was evaluated before treatment and 1 week after the end of the third treatment.SPSS26 input and statistical processing were used.Results:1.In the VAS score, the VAS scores of the two groups before treatment,1 week after the first treatment and 1 week after the third treatment were compared,P < 0.01,suggesting that the treatment methods of the two groups can relieve neck pain. The VAS scores of the two groups were compared 1 week after the first treatment and 1 week after the third treatment,P< 0.01,suggesting that the experimental group can significantly relieve neck pain in the treatment of cervical spondylosis.2.In the NDI index, the VNDI index of the two groups before treatment,1 week after the first treatment and 1 week after the third treatment,P < 0.01,suggesting that the treatment methods of the two groups can improve the neck function of patients with cervical spondylosis of cervical type. One week after the third treatment, the NDI index of the two groups was compared,P < 0.05,suggesting that the experimental group can significantly improve the cervical function in the treatment of cervical spondylosis.3.Among the SR values of the posterior cervical muscles, the SR values of the posterior cervical muscles of the two groups before treatment and one week after the third treatment were compared, and the SR values of the posterior cervical muscles of the experimental group were compared. The SR values of trapezius muscle and capitis muscle in the control group were all < 0.01,and the SR values of cephalic semispinalis muscle, cervical semispinalis muscle and multifida muscle showed a downward trend,P > 0.05,suggesting that the test group could significantly improve the hardness of posterior cervical muscle, while the control group could improve trapezius muscle and head clamp muscle, and there was no significant difference in head semispinalis muscle, cervical semispinalis muscle and multifida muscle before and after treatment. One week after the third treatment, the SR value of the posterior cervical muscle group of the two groups was compared(P < 0.05),suggesting that the experimental group was superior to the control group in improving the hardness of the posterior cervical muscle group.4.In the ultrasonic imaging study, the elastic images of the muscles behind the neck of the two groups before treatment and 1 week after the third treatment were compared, the blue area was less than before, and the red area was red.The green area increased, suggesting that the hardness of the posterior cervical muscle decreased in both groups.5.In the overall curative effect evaluation, the experimental group was markedly effective in 15 cases, effective in 10 cases, ineffective in 3 cases, markedly effective in 53.6%,total effective rate in 89.3%,markedly effective in 7 cases, effective in 16 cases, ineffective in5 cases, markedly effective in 25.0%,and the total effective rate was 82.1%(P < 0.05).It is suggested that the curative effect of the experimental group in the treatment of cervical spondylosis is better than that of the control group. Conclusions:Acupotomy deep acupuncture release compared with acupotomy shallow acupuncture release can significantly reduce pain symptoms, improve cervical function, reduce cervical muscle hardness and improve clinical efficacy in patients with neck type cervical spondylosis.
