| Objective:To systematically evaluate the outcome index of Bushen Huoxue decoction or proprietary Chinese medicine preparation by meta analysis,so as to provide reliable,scientific and objective basis for clinical treatment of lumbar disc herniation.Methods:1.Search CNKI database,Wanfang database,VIP database,The Cochrane library,SCI,Pub Med and other databases.The literatures on clinical randomized controlled trials of traditional Chinese medicine prescriptions for tonifying kidney and promoting blood circulation and proprietary Chinese medicine preparations(including journals,degree papers and conference papers)were screened,and the search time limit was from the establishment of the database to October 2019.The key words of Chinese literature are: “Bu Shen Huo Xue”、“Bu Shen Qu Yu”、“Yi Shen Huo Xue”、“Lumbar disc herniation”、“lumbar intervertebral disc herniation”、“Random”.English key words include: "Bu Shen Huo Xue","Bu Shen Qu Yu","Yi Shen Huo Xue","Lumbar disc herniation","lumbar intervertebral disc herniation","Random" and other key words.2.he retrieved documents are imported into Note Express software to eliminate duplicate documents,and then two people read the abstract and the full text independently according to the inclusion and exclusion criteria,extract data,make literature feature table,quality evaluation,data analysis,result interpretation and so on.Results:All the 29 studies included were randomized controlled trials with a total of 3465 patients with lumbar disc herniation,with a maximum sample size of 754 and a minimum sample size of 40.1.A total of 27 studies compared the total effective rate data,the results showed that the total effective rate of Bushen Huoxue method was higher than that of routine treatment group(RR=1.18,95%CI1.13to1.24,P < 0.00001).A total of 16 studies compared the VAS score data.The results showed that the pain relief effect of kidney-tonifying and blood-activating therapy in LDH was better than that in routine treatment group(MD=-1.27,95%CI-1.47to-1.08,P <0.00001).A total of 10 studies compared the JOA score data.The results showed that the JOA score of LDH treated by tonifying kidney and activating blood circulation was higher than that of routine treatment group(MD=3.16,95%CI2.43to3.89,P < 0.00001).A total of three studies compared the data of ODI score.The results showed that the treatment of tonifying kidney and activating blood circulation was superior to the routine treatment group in reducing ODI score of LDH(MD=-5.79,95%CI-9.52to-1.96,P=0.003 < 0.05).A total of three studies compared the IL-6 data.The results showed that the treatment of tonifying kidney and activating blood circulation was superior to the routine treatment group in reducing IL-6 in LDH(MD=-12.66,95%CI-24.90to-0.42,P=0.04 < 0.05).A total of three studies compared the data of IL-1 β.The results showed that the method of tonifying kidney and activating blood circulation was superior to routine treatment group in reducing IL-1 β in LDH(MD=-0.12,95%CI-0.14to-0.10,P < 0.0001).A total of two studies reported the comparison of TNF-α data.The results showed that the treatment of tonifying kidney and activating blood circulation was superior to the routine treatment group in reducing TNF-α in LDH(MD=-0.27,95%CI-0.36to-0.17,P < 0.0001).2.Adverse reactions of tonifying Kidney and activating Blood Circulation in the treatment of LDH.12 studies reported the adverse reactions of tonifying the kidney and promoting blood circulation in the treatment of LDH.The results showed that there was no significant difference in the adverse reactions between the kidney-tonifying and blood-activating therapy and the routine treatment group(OR=0.64,95%CI0.40to1.04,P=0.07 > 0.05).Conclusion:In the treatment of lumbar disc herniation,the total effective rate,VAS score,JOA score,ODI score,hospital stay,IL-6,IL-1 β,TNF-α and other aspects of Bushen Huoxue traditional Chinese medicine prescription or proprietary Chinese medicine preparation were superior to those of the control group in the treatment of lumbar disc herniation.Bushen Huoxue traditional Chinese medicine prescription or proprietary Chinese medicine preparation in the treatment of lumbar disc herniation are safer.There was no significant difference in adverse reactions between Bushen Huoxue traditional Chinese medicine prescription or proprietary Chinese medicine preparation in the treatment of lumbar disc herniation compared with the routine treatment group.Although the adverse reactions were reported in clinic,the adverse reactions were mainly concentrated in allergy,urticaria,nausea,somnolence and so on,but there were no serious adverse reactions and the safety of clinical application was high. |
