Objective:The TCM syndromes of sepsis are divided into toxic heat syndrome,blood stasis syndrome and acute deficiency syndrome.The purpose of this study was compare the differences of the three syndromes in immune and inflammatory indicators and the correlation with SOFA score through the detection of T lymphocyte subsets,PCT,LAC and other laboratory indicators.Methods:According to the inclusion criteria,103 patients diagnosed with sepsis in the emergency department,intensive care unit,respiratory department and other departments of our hospital from March 2019 to December 2020 were collected.After admission,the general data of patients in line with the diagnosis of sepsis were recorded.The dynamic and venous blood was collected and detected by the laboratory department of our hospital.The T lymphocyte subsets count,PCT and lactic acid(LAC)values were recorded,and SOFA scoring was performed to collect data.According to the collected data and the classification standard of traditional Chinese medicine,they were divided into toxic heat syndrome,blood stasis syndrome and acute deficiency syndrome.According to the western medical diagnostic standard of sepsis,they were divided into septic shock group and sepsis group.To observe the correlation between toxic heat syndrome,blood stasis syndrome,acute deficiency syndrome and T lymphocyte subsets count,PCT,LAC and SOFA score,Western medicine classification and 28-day mortality.SPSS 26.0 statistical software was used for data analysis.Results:1.A total of 103 cases were collected,including 35 patients(34.0%)in the poisoned heat syndrome group,30 patients(29.1%)in the blood stasis syndrome group,and 38 patients(36.9%)in the acute deficiency syndrome group.2.There were no statistical differences in age and gender among different syndromes(P>0.05).3.There were statistically significant differences in T lymphocyte subsets count among different syndromes(P<0.05).According to one-way ANOVA,there were significant differences in CD4+,CD8+and CD4+/CD8+among the toxic-heat syndrome group,the blood-stasis syndrome group and the acute deficiency syndrome(P<0.05).4.There were significant differences between different syndromes and PCT and LAC levels(P<0.05).The levels of PCT and LAC in acute deficiency syndrome were significantly higher than those in toxic heat syndrome and blood stasis syndrome(P<0.05),with statistical significance.5.There were significant differences in SOFA score,Western medicine classification and28-day mortality among different syndromes(P<0.05).SOFA score from high to low were acute deficiency syndrome,blood stasis syndrome and toxic heat syndrome.6.TCM syndromes were positively correlated with SOFA score,28-day mortality rate,PCT and LAC value.The syndromes of TCM were negatively correlated with CD4+and CD4+/CD8+values,and positively correlated with CD8+values.Conclusion:1.CD4+and CD4+/CD8+in acute deficiency syndrome group were significantly lower than those in toxic heat syndrome and blood stasis syndrome group.2.PCT and LAC values of toxic heat syndrome,blood stasis syndrome and acute deficiency syndrome showed a gradually increasing trend,and the infection and tissue ischemia and hypoxia of acute deficiency syndrome were the most serious.3.SOFA score is the highest for acute deficiency syndrome,which is more prone to multiple organ failure than other syndrome types,with more serious conditions and higher mortality.4.Among the traditional Chinese medicine syndrome types of sepsis,the severity of acute deficiency syndrome is the most serious,so the treatment should pay attention to replenishing vital qi and strengthening the foundation。...
