Objective:By obtaining serum differential metabolites of people with spleen,liver,and kidney disease location with phlegm syndrome of metabolic syndrome(MS),distinguishing the metabolic profile,exploring possible biomarkers and key metabolic pathways in different locations of MS with phlegm syndrome,and analyzing the metabolomics commonality and individual characteristics of different disease location of MS phlegm syndrome.it provides a reference for the early detection,diagnosis and treatment of MS phlegm syndrome.Methods:Collecting the basic information,four-diagnosis information and blood sample of MS patients who meet MS diagnostic criteria and healthy people in the same period,MS patients were divided into 3 groups of MS phlegm syndrome in disease location of spleen,liver,and kidney using the method of “syndrome differentiation”.We separate the serum from the blood sample and using UHPLC-QE-MS technology to detect the serum metabolites of patients who with MS phlegm syndrome in different disease location and healthy people.A pairwise comparison between the diseased group and the healthy group is carried out,and then the differential metabolites of each diseased group and the healthy group are compared twice to obtain the possible biomarkers for different disease locations of MS phlegm syndrome.Log in to the Kyoto Encyclopedia of Genes and Genomes(KEGG)database to sort out all the pathways mapped by each group of differential metabolites,and through comprehensive analysis(including enrichment analysis and topological analysis)of the pathways where the differential metabolites are located,to find the key metabolic pathways in different locations of MS phlegm syndrome.Results:1.Comparing the group of spleen with MS phlegm syndrome and the healthy group,with VIP>1,P<0.05 as the limit value,270 different metabolites were fin-ally identified,of which VIP≥2,P<0.05 there are 5 kinds of significantly different metabolites in the positive ion mode,they are L-Valine,Piperidine,DL-Glutamate,LGlutamic acid,PC(16:0/P-16:0),While there are 11 metabolites in negative ion mode,namely Oleic acid,Pyroglutamic acid,Xanthine,D-Mannose,L-Tyrosine,Orotidine,2-Ketobutyric acid,L-Fucose,Methylsuccinic acid,D-Glucuronic acid,Galacturonic acid.2.Comparing the group of liver with MS phlegm syndrome and the healthy group,with VIP>1,P<0.05 as the limit values,256 different metabolites were finally identifie-d,of which VIP≥2,P<0.05,there are 7 kinds of significantly different metabolites in the positive ion mode,they are Choline,L-Phenylalanine,DL-Glutamate,L-Glutamicacid,Glycerophosphocholine,(3xi,6xi)-Cyclo(alanylvalyl),Lyso PC(16:0),While there are 10 metabolites in negative ion mode,namely Pyroglutamic acid,Succinic acid,Ino-sine,Malonic acid,Guanosine,Lyso PA(16:0/0:0),Citric acid,Citraconic acid,Cytidine,Gala-cturonic acid.3.Comparing the group of kidney with MS phlegm syndrome and the healthy gro-up,with VIP>1,P<0.05 as the limit values,214 different metabolites were finally ide-ntified,of which VIP≥2,P<0.05,there are 10 kinds of significantly different metabo-lites in the positive ion mode,they are(2R,3R,4R)-2-Amino-4-hydroxy-3-methylpentan o-ic acid,Pivaloylcarnitine,PC(16:0/P-16:0),PC(16:1(9Z)/P-18:1(11Z)),PC(18:1(9Z)/P-18:1(11Z)),PC(18:0/P-16:0),PC(22:5(4Z,7Z,10 Z,13Z,16Z)/P-18:0),PC(22:4(7Z,10 Z,13Z,16Z)/P-18:0),PC(16:1(9Z)/P-18:0),Lyso PC(24:0),While there are 5 metabolites in negat-ive ion mode,namely Succinic acid semialdehyde,2-Keto-3-deoxy-D-gluconic acid,2-Ket-obutyric acid,L-Fucose,Methylsuccinic acid.4.Compare the 270,256,and 214 differential metabolites produced by the MS phlegm syndrome of spleen,liver,and kidney disease location group and the healthy group to obtain the different differential metabolites between the disease location groups,and then compare the different differential metabolites.After the second comparison,27,32 and 36 possible biomarkers of spleen,liver,and kidney disease location were finally excavated.5.The differential metabolic pathways obtained by comparing MS phlegm syndrome of spleen,liver,and kidney disease location groups with the healthy group were compared twice,and the unique metabolic pathways of the three groups of disease locations were found.The results found that the specific metabolic pathways in the spleen,liver,and kidney groups of MS phlegm syndrome were biotin metabolism,steroid hormone synthesis,and fatty acid metabolism.Conclusion : 1.The common feature of MS phlegm syndrome in different disease location of spleen,liver,and kidney are that the body is mainly composed of lipid,amino acid,and glycerophospholipid metabolism disorders.2.The individual characteristics of MS phlegm syndrome different disease location of spleen,liver,and kidney have 27,32,and 36 characteristic biomarkers respectively.These biomarkers may be the objective material basis for the spleen,liver,and kidney diseases location of MS phlegm syndrome.3.Biotin metabolism,steroid hormone synthesis,and fatty acid metabolism may be used as breakthrough points to explore the connotation of the spleen,liver,and kidney diseases location of MS phlegm syndrome.
