| Purpose:Dual axis rotational coronary angiography(DARCA)is a new technique for coronary angiography.This study was to compare the clinical application and radiation dose in DARCA and conventional coronary angiography(CCA).Methods:The data of 190 patients undergoing CCA were analyzed retrospectively.Effective dose(ED)of patients was computed by using effective dose conversion factor.The correlation of the effective dose was analyzed by body mass index(BMI),the approach,fluoroscopy time,number of frames and vessel disease.Application of the Chinese anthropomorphic chest phantom,virtual surgical environment of DARCA and CCA were simulated,the thermo luminescent dosimeters(TLD)were placed on its major organs,the organs’absorbed doses were measured and the effective doses were computed.30 cases(20 cases of CCA,10 cases of DARCA)of coronary angiography were compared in the clinical,the fluoroscopy time,contrast agent usage,radiation dose and the effect of clinical application were analyzed in two groups.Results:The average dose of the patients was 400.54m Gy,the average dose-area product(DAP)was 28.26Gy·cm~2,the average ED was 5.23m Sv.The regression coefficient of fluoroscopy time,BMI and number of frames was 0.757,0.442 and 0.012,respectively.There was significant difference(P<0.05).The mean radiation doses in DARCA are lower than CCA(p=0.03);the effective doses in DARCA are lower than CCA(4.94m Sv vs.5.57m Sv).The fluoroscopy time,contrast agent usage and radiation dose were 3.76 min,29.70 ml,15.57Gy·cm~2in DARCA,and 4.27min,41.10ml,24.07 Gy·cm~2in CCA.It is similar to the clinical evaluation of the two technologies.Conclusion:Obesity,longer fluoroscopy time and more number of frames are significant factors for higher radiation dose for CCA,the radiation dose can be reduced by highly skilled operators and the technology that can bring down the number of frames.DARCA can fulfill the image quality and reduced contrast agent usage and radiation dose effectively in coronary angiography. |
