| Research Purpose:1.Construct a nursing intervention program based on fear avoidance model,and analyze the impact of the program on patients with unilateral total knee arthroplasty from multiple outcome indicators such as compliance scale of functional exercise for orthopedic patients score,knee flexion and hospital for special surgery score(HSS);2.To explore the application value of nursing intervention program based on fear avoidance model in patients with unilateral total knee arthroplasty.Research Methods:1.In this study,a nursing intervention program based on the fear avoidance model was constructed by referring to a large number of literatures,clinical situations and characteristics of patients with unilateral total knee arthroplasty,and the final intervention program was formed through expert consultation,modification and finalization.2.82 patients with unilateral total knee arthroplasty in a tertiary hospital of Huzhou from January 2020 to February 2021 were selected for the experiment,and randomly divided into control group and experimental group.Control group given routine nursing care,the experimental group was given nursing intervention program based on fear avoidance model,two groups of patients on the basis of information collection,after operation 3 days,7 days and one month were collected compliance scale of functional exercise for orthopedic patients score,knee flexion(ROM),hospital for special surgery(HSS)score.All the collected data were double-entered and checked using Excel.After confirmation,SPSS Statistics 23.0 was used for statistical analysis,and P < 0.05 showed statistical difference.Research Results:1.The general data included age,gender,body mass index(BMI),education level,marital status,work,diabetes,hypertension,smoking,etc.there was no significant difference between the two groups(P>0.05);the disease-related data included pain years,pain scores,pain knowledge scores,surgical site,preoperative compliance scale of functional exercise for orthopedic patients scores,reoperative knee flexion(ROM),preoperative hospital for special surgery scores(HSS),Tampa scale of kinesiophobia scores(TSK),pain disaster rating scale scores,and so on,there was no statistically significant difference between two groups(P>0.05).2.The mean scores of compliance scale of functional exercise for orthopedic patients in the two groups were as follows: the experimental group(48.83±5.77)and the control group(40.90±5.06)on the third day after operation,P < 0.001;On the 7th day after operation,they were:experimental group(59.88±9.86),control group(46.83±8.79),P < 0.001;The first month after operation were: experimental group(67.05±5.36),control group(56.85±9.53),P < 0.001.3.The mean value of knee flexion in the two groups on the third day after operation were: the experimental group(84.27±12.73)and the control group(77.93±10.58),P=0.016 < 0.05;On the7 th day after operation,the experimental group(99.37±5.56)and the control group(95.78±6.19)were respectively(P=0.007<0.05);One month after operation,the experimental group(118.17±5.36)and the control group(115.78±4.90)were respectively(P=0.038<0.05).4.The mean value of hospital for special operation scores in the two groups on the third day after surgery were as follows: experimental group(57.68±4.37)and control group(54.10±4.88),P=0.001< 0.05;On the 7th day after operation,the experimental group(68.17±4.09)and the control group(64.37±3.57)were respectively(P< 0.001);The first month after operation were as follows: experimental group(74.61±6.00),control group(70.78±6.13),P=0.005< 0.05.Research Conclusion:1.The experimental group and the control group were compared and analyzed from the scores of compliance scale of functional exercise for orthopedic patients,knee flexion(ROM)and hospital for special surgery scores(HSS),and the results of the experimental group were more satisfactory.so can conclude nursing intervention program based on fear avoidance model can effectively improve the postoperative early rehabilitation exercise in patients with unilateral total knee arthroplasty compliance,thus promote the functional recovery of knee joint;2.The nursing intervention program based on fear avoidance model is feasible in clinical work and has certain clinical application value,providing new thoughts for clinical workers. |
