Objective1.To explore the effect of age group,gender and location on MR quantitative measurement indexes of normal coracoid process.2.To investigate the value of MR quantitative measurement indexes in diagnosis of subcoracoid impingement syndrome(CIS).Materials and Methods1.The MR images of 100 cases of normal shoulder were analyzed retrospectively.The effects of age group,gender and location on MR quantitative measurement indexes of normal coracoid process were explored.MR quantitative measurement indexes included the coracoid humeral distance(CHD),coracoid index(CI),height of lesser tuberosity(HLT),coracoid obliquity(CO),coracoglenoid angle(CGA),coracohumeral angle(CHA),width of the subscapular tendon(WST),contact distance between subscapular tendon and coracoid process(CD).2.47 cases of CIS were enrolled in this study.The values of CHD,CI,HLT,CO,CGA,CHA,WST,CD in diagnosis of CIS were investigated.Results1.In the normal group,the mean values of CHD,CI,HLT,CO,CGA,CHA,WST,CD were 7.67±1.53 mm,15.98±3.28 mm,5.44±0.95 mm,8.90±2.94°,132.60±8.51°,106.65±9.54°,5.35±1.32 mm,8.94±2.25 mm respectively.The CI in older group was significantly longer than that of younger group(P=0.024).The CHD,HLT,CGA,WST,CD in the male group were significantly greater than those in the female group(P=0.001,P=0.008,P<0.001,P=0.037,P=0.003).The CHA in the male group was significantly less than that in the female group(P=0.020).The CO,CGA,CHA in the right side were significantly greater than those in the left side(P=0.028,P=0.022,P<0.001).2.In the CIS group,CHD,CI,HLT,CO,CGA,CHA,WST and CD did not show difference between the older group and the younger group(P>0.05).The HLT and CD in the male group were significantly greater than those in the female group(P=0.013,P=0.001).The HLT in the left side was significantly greater than that in the right side(P=0.001);the CGA,CD in the left side were significantly less than those in the right side(P=0.010,P=0.033).The CHD,CGA of the CIS group were significantly less than those of the normal group(P<0.001,P<0.001);the CI,HLT,CHA,WST,CD were significantly greater than those of the normal group(P<0.001,P<0.001,P=0.027,P<0.001,P=0.026);CO did not show difference between the CIS group and the normal group(P=0.567).Receiver operating characteristic curve(ROC)of CHD on diagnosing CIS showed the area under the curve(AUC)was 0.963.AUC of combined CHD and HLT was0.986,which was higher than that of CHD(P=0.036).CHD showed positive correlation with CGA(P=0.008)and negative correlation with CI(P=0.012)and CHA(P=0.007).Conclusions1.CHD,CI,HLT,CO,CGA,CHA,WST,CD were influenced by age group,gender and location in the normal group.2.CHD,CI,HLT can be used as the quantitative diagnostic indexes of CIS,of which CHD has the highest diagnostic efficacy.Moreover,CHD combined with HLT shows the higher efficiency and clinical practicability.In clinical application of these MR indexes,the effect of gender,location should be considered.
