| Objective:The effect of digital technique in patients with cleft jaw deformity was evaluated by comparing the changes of soft and hard tissue before and 6 months after operation.Methods:In this study,16 patients were diagnosed as hemifacial deformities,all of them had complete growth and development and had undergone preoperative orthodontic treatment,which met the indication of orthognathic surgery.CT examination of the head and neck was performed one week before operation and 6 months after operation.The CBCT data of the patient’s head were input into mimics 20.0 software in DICOM format for three-dimensional reconstruction of the skull,and the independent upper and lower dentition were reconstructed respectively.Then the laser scanning dental mold data were registered with the dentition model reconstructed by CT three-dimensional reconstruction,so as to realize the fitting between them.Finally,a three-dimensional image reflecting the jaw shape and dentition occlusion of patients with dental and maxillo facial deformities was obtained.On the reconstructed enhanced skull model,the mirror technique is used to establish an accurate three-dimensional reference plane,then the bone tissue landmarks are selected,the measurement items are determined,the appropriate operation plan is made according to the measurement results,and the operation simulation is carried out in the software.after the simulation is completed,the symmetry of the mandible structure is analyzed according to the mirror technique to determine whether bone contour repair or secondary surgery is needed.According to the design,the operation was completed,and then the postoperative CT data were imported into the software for three-dimensional reconstruction measurement.SPSS 26.0 was used to analyze and compare the data before and after orthognathic surgery.The postoperative effect was evaluated by mirror technique and patients’ subjective satisfaction.Results:1.The significance of paired sample t test for U1-SP、U3-FH、U6-FH、U3-SP、U6-SP、Me-SP、Go-FH、L6-SP before and after surgery was P<0.05,indicating that these variables were significantly different before and after surgery,and the value of postoperative variables was significantly lower than that before surgery,indicating that there were significant changes in bone tissue of patients after surgery.2.The results of paired t-test on the left and right sides of facial bone tissue of 16 patients after operation,P > 0.05,the difference was not statistically significant,and it was considered that the left and right sides of facial bone tissue of 16 patients were basically symmetrical after operation.3.The application of digital technology in the correction of patients with partial jaw deformity was performed smoothly and there were no complications such as massive hemorrhage and fracture.4.Postoperative follow-up observation showed that the facial soft tissue of the patient recovered well and the effect of the operation was satisfactory.Preoperative and postoperative soft tissue overlap color contrast color map shows that the postoperative soft tissue has changed,and the postoperative mirror image shows that the symmetry of facial soft and hard tissue is good.It shows that digital technology-assisted preoperative design can effectively guide the implementation of surgery for herniated jaw deformity.Conclusion:1.Digital technology helps surgeons to visualize soft and hard tissues.2.With the help of digital technology,orthognathic surgery can improve the appearance of patients with eccentric jaw deformity and better meet the needs of patients. |
