Objective: By observing the infrared thermogram of different TCM syndrome differentiation types of patients with femoral head necrosis,measuring and analyzing the temperature difference of the observation site,and then looking for the specific observation area and corresponding temperature change corresponding to the infrared thermography of different syndrome types of femoral head necrosis,to provide objective quantitative reference indexes for better TCM clinical syndrome differentiation and treatment of femoral head necrosis.Methods: From September 2018 to December 2019,patients with necrosis of the femoral head who met the inclusion criteria of different TCM Syndrome Types and volunteers who met the requirements were collected from the outpatient department of Henan orthopedic hospital,and all of them were tested by using sp-9000 medical infrared thermal imager(Taiwan Hantang company).Each syndrome type(including normal volunteers)was tested by using random digital table method Group)30 cases were selected respectively,and the temperature difference changes of specific parts were collected.The difference was statistically analyzed among the groups of different TCM Syndromes of femoral head necrosis and between each classification and the normal control group.Results:1.There was no significant difference in sex,age,BMI and Arco stages between the observation group and the control group,as well as in the three groups of the observation group(P > 0.05);the VAS score in the observation group was significantly lower than that in the qi stagnation and blood stasis group(4.9 ± 0.6 in the phlegm stagnation and blood stasis group,5.5 ± In the observation group,the VAS score was significantly lower than that in the kidney deficiency and blood stasis group(4.9 ± 0.6 in the phlegm stasis group,5.4 ± 0.7 in the qi stagnation and blood stasis group;P < 0.05);in the observation group,the VAS score was significantly lower than that in the Qi stagnation and blood stasis group(5.4 ± 0.7 in the kidney deficiency and blood stasis group,5.4 ± 0.7 in the qi stagnation and blood stasis group;P < 0.05);2.There are differences in the temperature of popliteal fossa(disease),hypochondriac costal part,Blood Sea(disease),epigastric part,lower part of back,and circumflex(disease)area among the groups.3.The temperature of the epigastric region was the lowest and lower than that of the normal group,and the difference was statistically significant(P < 0.05);the temperature of the costal region was the highest and the difference was statistically significant(P < 0.05).4.The area of popliteal fossa(affected area),Xuehai point(affected area),lower back area,costal area,ring jump(affected area)were significantly correlated with pain score.5.There was significant correlation between Arco and blood sea area,lower back area,costal area,epigastric area,ring jump area.6.In the phlegm and blood stasis group: there was a significant correlation between the stomach cavity and Arco(r =-0.254,P < 0.05);the lower the temperature of the stomach cavity,the more serious the condition of Arco stage,the better the reaction of Arco stage in the phlegm and blood stasis group;7.In Qi stagnation and blood stasis group: Huantiao point has a significant correlation with Arco(r =-0.233,P<0.05);8.In the kidney deficiency and blood stasis group: there was a significant correlation between blood sea and Arco(r = 0.258,P<0.05);9.In the phlegm and blood stasis group,there was a significant correlation between the pain score and the area of the stomach cavity(r =-0.263,P<0.01);10.In the qi stagnation and blood stasis group,there was a significant correlation between the costal area and the pain score(r = 0.337,P < 0.01): the higher the temperature of costal area was,the higher the pain score of the patients was,the better the response of the costal area to the pain score of the qi stagnation and blood stasis group was.11.In the kidney deficiency and blood stasis group,there was a significant correlation between the blood sea area and pain score(r= 0.525,P<0.01);Conclusion:1 Infrared imaging technology has certain guiding significance in the diagnosis of femoral head necrosis.2 Infrared imaging technology has certain guiding significance for the classification of different syndromes of osteonecrosis of the femoral head.
