Background:Proton pump inhibitors(PPIs)are widely used in clinical practice,but its adverse reactions have been reported more and more in recent years.Short-term use of PPIs increases risk of acute kidney injury(AKI),as do long-term oral PPIs for the onset and progression of chronic kidney disease to end-stage kidney disease.Research objective: the main purpose of this study was to retrospectively analyze the effect of proton pump inhibitors on the progression of disease in patients with chronic kidney disease in Yinzhou district,Ningbo city,so as to provide reference for the clinical use of such patients.Methods: a retrospective case-control study was conducted using the database provided by the center for disease control,prevention and control of Yinzhou district,Ningbo city.The target population was patients with confirmed chronic kidney disease.The patients were matched according to gender,age,complications,stage of chronic kidney disease,and history of proton pump inhibitor use.Cox regression models were used to estimate the odds ratio(OR value)associated with the duration of the disease and the use of proton pump inhibitors.Results:CKD patients with PPIs had a 1.12-fold increased risk of doubling their serum creatinine or progressing to ESRD compared with those without PPIs.And all PPIs drugs involved in this study,including omeprazole,lansoprazole,pantoprazole,rabeprazole,and esomeprazole(esomeprazole),were associated with an increased risk of renal decline in CDK patients.In addition,the risk effect values of CKD patients with different gender,age,CKD stage,complications,duration and dose of PPIs were different.Conclusion: for patients with chronic kidney disease,clinicians should be more cautious in the use of proton pump inhibitors(ppis).
