| Objective:To explorethe correlation between common laboratory indicators and the severity of hyperlipidemic acute pancreatitis and the predictive value on the severity of its.Methods:A total of 171 patients with hyperlipidemic acute pancreatitis admitted to the Gastroenterology Department of the Affiliated Hospital of North Sichuan Medical College from January 2019 to January 2021 were selected.According to the guidelines for the diagnosis and treatment of acute pancreatitis in Shenyang,China,in 2019,they were divided into two groups according to the severity of the disease:non-severe hyperlipidemic acute pancreatitis group and severe hyperlipidemic acute pancreatitis group.To collected the general data and common laboratory indicators of the two groups,for retrospectively analyzed the clinical data characteristics of the two groups,At the same time,receiver operating characteristic(ROC)curve was used to evaluate the value of the laboratory indicators in predicting the severe hyperlipidemic acute pancreatitis.Results:1 Two groups of laboratory index of single factor analysis showed that intensive hyperlipidemia sex acute pancreatitis group of mean platelet volume,aspartate amino transferase,triglycerides,log(TG/HDL-C),fibrinogen degradation products,lactic acid is higher than the severe acute pancreatitis group of hyperlipidemia,statistically significant difference,P<0.05;2 The correlation study between the two groups of statistically significant indicators and the severity of HLAP showed that average platelet volume,aspartate aminotransferase,triglyceride,log(TG/HDL-C),fibrinogen degradation products,lactic acid were positively correlated with the severity of hyperlipidemic acute pancreatitis.The correlation coefficients(r)were respectively 0.174,0.260,0.181,0.193,0.224,0.224,0.244.All above were statistically significant(P<0.05).3 Multivariate binary Logistic regression analysis of risk factors for severe hyperlipidemic acute pancreatitis showed that log(TG/HDL-C),lactic acid and aspartic acid aminotransferase were independent risk factors for severe hyperlipidemic acute pancreatitis,with statistical significance(P<0.05).4 The ROC curve of predicting the severity of acute pancreatitis with hyperlipidemia by various indexes showed that the area under the ROC curve(AUC)of aspartic acid aminotransferase in predicting severe acute pancreatitis with hyperlipidemia was the largest(A UC=0.751).The AUC of log(TG/HDL-C),lactic acid,fibrinogen degradation products,triglyceride,high density lipoprotein cholesterol and mean platelet volume in predicting severe hyperlipidemic acute pancreatitis were 0.686,0.735,0.716,0.675,0.399 and 0.668,respectively.The AUC of lactic acid combined with aspartate aminotransferase in the prediction of severe hyperlipidemic acute pancreatitis was 0.786.All the above results were statistically significant(P<0.05).Conclusion:The levels of lactic acid,aspartate aminotransferase and log(TG/HDL-C)are closely related to the severity of acute hyperlipidemic pancreatitis,and they are independent risk factors for early severe acute hyperlipidemic pancreatitis.It can be used to predict severe hyperlipidemic acute pancreatitis. |
