Aims:We detect the expression of Lgr5 protein to explore the mechanism and clinical significance of Lgr5 protein in colorectal laterally spreading tumor.Methods:We collected 30 cases of laterally spreading tumors(LST)and 30 cases of protruded-type colorectal adenoma(PA)in the First People’s Hospital of Chenzhou from June 2016 to June 2020,and We selected 47 pairs of cases of colorectal cancer(CRC)and normal colorectal mucosa from a tissue microarray chip.We gathered the clinicopathological data of these samples meanwhile.LST group were divided into three types:granular homogeneous type(LST-GH),nodular mixed type(LST-GM)and non-granular type(LST-NG).The expression of Lgr5 protein in each group was detected by immunohistochemical method,and the data were analyzed by SPSS 25.0.Results:1.In the normal colorectal mucosa group,Lgr5~+staining cells were sporadically distributed at the bottom of the crypt,PA and LST showed staining on the crypt and glandular surface,but the crypt area was more and largely stained,while CRC showed deeper staining of the whole crypt and glandular surface.2.The positive expression rates of Lgr5 in normal colorectal mucosa group,PA group,LST group and CRC group were 3.33%(1/30),26.67%(8/30),46.67%(10/30),76.60%(36/47),respectively.There was significant difference among the three groups(P<0.05).3.The expression rate and expression intensity of Lgr5 in LST group were higher than those in normal colorectal mucosa group(P<0.05).4.There was no significant difference in the ratio and intensity of Lgr5 between LST group and PA group(P>0.05).Subgroup analysis showed that there was no significant difference in the expression rate and intensity of Lgr5 among LST-GH group,LST-GM group and PA group(P>0.05).The expression rate and intensity of Lgr5 in LST-NG group was significantly higher than that in PA group(P<0.05).5.The expression rate and intensity of Lgr5 in LST group were significantly lower than those in CRC group(P<0.05).Subgroup analysis showed that the expression rate and intensity of Lgr5 protein in LST-GH group and LST-GM group were significantly lower than those in CRC group(P<0.05).There was no significant difference in Lgr5 protein expression and expression intensity between LST-NG group and CRC group(P>0.05).Conclusions:1.The Lgr5 staining area of LST increased gradually from the bottom of the crypt to the glandular surface,and this expression pattern may be related to the stem cell mutation at the bottom of the colorectal crypt.2.There was no significant difference in the expression of Lgr5 between LST-GH group,LST-GM group and PA group,but it was significantly lower than that in CRC group,suggesting that Lgr5 plays a similar role in the lesions of LST-GH,LST-GM and PA,and the carcinogenesis potential of LST-GH and LST-GM is relatively low.3.The expression of Lgr5 in LST-NG type is significantly higher than that in PA group,but there is no significant difference between LST-NG type group and CRC group,so Lgr5 may be related to the higher malignant potential of LST-NG type.
