Objective: To analyze the relationship between US,MRI and clinicopathological of breast cancer and intravascular tumor thrombus,and to provide help for the treatment and prognosis of breast cancer.Methods: The ultrasonography,MRI findings and clinicopathological results of 236 breast cancer patients admitted to Nanhua Hospital and First Affiliated Hospital of University of South China were analyzed.Patients were divided into group A(146 cases)with intravascular tumor thrombus and group B(90 cases)without intravascular tumor thrombus.Results: 1.The pathological type(p<0.001),pathological grade(p=0.033)and axillary lymph node metastasis(p<0.001)of breast cancer were notability different between the two groups,there was no notable difference between the two groups in age,immunomolecular typing,number of ALN metastasis/total metastasis,ER,CK5/6,PR,P120,HER-2 and Ki-67.2.There were notable differences between the two groups of the ultrasonographic features include hyperechoic halo(p<0.001),posterior echo attenuation(p =0.036),burr(p <0.001),axillary lymph node metastasis(p <0.001),BI-RADS classification(p <0.001),and blood flow classification(p <0.001),there were no notable differences in echo,shape,boundary,calcification and aspect ratio between the two groups.3.The difference of suspected lymph node metastasis(p < 0.001)after MRI enhancement between the two groups which was statistically significant,there was no notable difference in tumor enhancement type,enhancement curve and DWI signal between the two groups.4.Intravascular tumor thrombus was positively correlated with burr,hyperechoic halo,and nodes metastasis(r=0.652,r =0.648,r=0.610).ntravascular tumor thrombus was positively correlated with suspected lymph nodes by MRI enhancement(r =0.534).Intravascular tumor thrombus was positively correlated with nodular blood flow grade,suspicious lymph nodes found by US,pathological grade(r =0.488,r=0.475,r =0.476).Intravascular tumor thrombus was positively correlated with pathological type,posterior echo attenuation and BI-RADS classification(r =0.139,r =0.137,r =0.362),with p < 0.05.Conclusion: 1.Hyperechoic halo,burr,blood flow grade,axillary lymph node ultrasound signs and MRI axillary lymph node enhancement characteristics can evaluate whether there is cancer thrombus in the vessel to a certain extent.2.The pathological grade of breast cancer and axillary lymph node metastasis were positively correlated with intravascular tumor thrombus.
