ObjectiveComparing with the treatment of acupuncture combined with pelvic traction, this study mainly explored the clinical efficacy and safety of acupuncture combined with fixed-point bone-setting in the therapy of lumbar disc herniation,to provide clinical data and ideas for optimizing the conservative treatment of LDH.MethodsEighty-six patients with LDH were randomly assigned to the treatment group and the control group. The patients in the treatment group treated LDH received acupuncture combined with fixed-point bone-setting,common acupuncture was performed 3 times a week, and fixed-point bone-setting was performed once a week. Those in the control group received acupuncture combined with pelvic traction. Both common acupuncture and pelvic traction were performed 3 times a week. The treatment of these two groups cost 4 consecutive weeks, and the total period of observation lasted for 28 weeks. The JOA score,VAS score, and index of lumbar spinal curvature(LCI)were used as the main outcomes data analyzed with SPSS23.0 to evaluate the efficacy and safety of these two combination therapies.Results1.A total of 86 patients were enrolled in the study,of which 84 were completed, including 43 in the treatment group with complete cases, and 41 in the control group with 2 drop-out cases.2.In the treatment group,19 patients were cured,14 patients were markedly effective,7 patients were effective, and 3 patients were ineffective. The total effective rate was 93.0%,and the good improvement rate was 76.7%;in the control group,6 patients were cured,14 patients were markedly effective,14 patients were effective, and 7 patients were ineffective. The total effective rate was 82.9%,and the good improvement rate was 48.8%.After testing, there was no statistically significant difference in the total effective rate between the two groups(P>0.05),but the difference in the good improvement rate was statistically significant(P<0.05),suggesting that the efficacy of the treatment group is better than the control group.3.The intra-group comparison of the JOA score:The treatment group and the control group gradually increased after 2 weeks and 4 weeks of treatment. After testing, the difference was statistically significant(P<0.05),suggesting that these two combination therapies can effectively improve the lumbar function of LDH patients.A 3-month follow-up showed that the JOA score has a downward trend in these two groups after stopping treatment, but still higher than that after 2 weeks of treatment, suggesting that the two treatment options can solidify the treatment effect. In comparison between the groups:Although the two treatment methods showed similar trends of efficacy during the whole course(early, midterm, the end of the treatment.and consolidation period),the treatment group had better efficacy than the control group from the early treatment period, and the JOA scores were higher than the control group all the time, the difference was statistically significant after testing(P<0.05).To sum up,the combination therapy of the treatment group has advantages in slowing down the progression and easing the effect of LDH.4.The VAS score was compared within the group. After 4 weeks of treatment, the VAS score of these two groups decreased compared with that before treatment, and the difference was statistically significant(P<0.01).In terms of the comparison between the two groups, the VAS score of the treatment group after treatment was lower than that of the control group, and the difference was statistically significant(P<0.05).The VAS score decreased in the treatment group was more significant than that of the control group, and the difference was statistically significant(P<0.01).All of these outcomes indicate the combination therapy of the treatment group is better than that of the control group.5.The intra-group comparison of the LCI figure:The LCI figures of the two groups increased compared with before the treatment. After testing, the difference was statistically significant(P<0.05).Comparing between the groups, the difference in LCI figures before and after the treatment was statistically different(P<0.05),suggesting that the combination therapy of the treatment group improves the sagittal curvature of the lumbar.6.The follow-up showed that at the 3-month follow-up,there were 2 cases of recurrence in the treatment group, with a recurrence rate of 4.7%,and 8 cases in the control group with a recurrence rate of 19.5%.The difference in recurrence rates between the two groups was statistically significant(P<0.05),indicating that the lasting effect of the treatment group is better than that of the control group.Due to the time limitation of this study, the long-term efficacy has yet to be further verified by follow-up.ConclusionBoth the treatment of acupuncture combined with fixed-point bone-setting and acupuncture combined with pelvic traction can improve the clinical symptoms, signs, and function of patients with LDH.However, the treatment of acupuncture combined with fixedpoint bone-setting is more effective, efficient, and safe than the treatment of acupuncture combined with pelvic traction. With the Low recurrence rate, simple and safe operation, and high treatment efficiency of the combination therapy of fixed-point bone-setting and acupuncture,it is worth popularizing and applying.
