| Objective:To explore the relationship between the morphological parameters of pterygium and corneal wavefront aberration,and the effect of pterygium surgery on corneal wavefront aberration,so as to determine the timing of pterygium surgery.Methods:According to the criteria of inclusion and exclusion,80 eyes of 59 primary pterygium patients who were treated in the Second Affiliated Hospital of Kunming Medical University from September 2020 to December 2020 were collected,of which 40 eyes of 34 pterygium patients were treated with pterygium resection and keratoconjunctival flap transplantation.The anterior segment of the cornea was taken by slit lamp anterior segment photography system,and the length,width and area of the pterygium invading the cornea were measured.The thickness of pterygium at the edge of the cornea was measured by optical coherence tomography(OCT),and the wavefront aberration of the patients before and after operation was recorded by OPD scan Ⅲ diopterometry.The relationship between the length,width,thickness,area of pterygium invasion and the wavefront aberration,the change of wavefront aberration before and after operation and the corneal wavefront aberration after operation were analyzed.The changes of the wavefront aberrations before and after operation were analyzed.The parameters of wavefront aberration include:astigmatism,total wavefront aberration,high-order aberration,coma aberration and trefoil aberration.Results:The length,width and area of pterygium invasion into cornea were significantly correlated with preoperative corneal astigmatism,total wavefront aberrations,higher-order aberrations,coma aberrations and trefoil aberrations(P<0.001).Pterygium thickness was also correlated with preoperative total wavefront aberrations,higher-order aberrations,coma aberrations and trefoil aberrations(P<0.05),but not with preoperative corneal astigmatism(P>0.05).The length of pterygium invading cornea was correlated with the changes of corneal astigmatism,total wavefront aberration,higher-order aberration,coma aberration and trefoil aberration before and after operation(P<0.05).The width of pterygium invading cornea was correlated with the changes of corneal astigmatism and total wavefront aberrations before and after surgery(P<0.05),but not with the changes of corneal higher-order aberrations,coma aberrations and trefoil aberrations before and after surgery(P>0.05).There was no correlation between the corneal thickness of pterygium invasion and the changes of corneal astigmatism,total wavefront aberration,higher-order aberration,coma aberration and trefoil aberration before and after surgery(P>0.05).The corneal area of pterygium was correlated with the changes of corneal astigmatism,total wavefront aberration,higher-order aberration and trilobal aberration before and after surgery(P<0.05),but not with the changes of coma aberration before and after surgery(P>0.05).The length of pterygium invading cornea was correlated with postoperative astigmatism(P<0.05),but not with postoperative total wavefront aberration,higher-order aberration,coma aberration and trilobal aberration(P>0.05).The width of corneal invasion was correlated with postoperative corneal astigmatism,total wavefront aberration,higher-order aberration,coma aberration and trefoil aberration(P<0.05).There was no correlation between pterygium thickness and all postoperative corneal wavefront aberrations(P>0.05).There was a correlation between the area of corneal invasion and postoperative corneal astigmatism,total wavefront aberration,higher-order aberration and trilobal aberration(P<0.05),but there was no correlation between the area of corneal invasion and postoperative coma aberration(P>0.05).One month after pterygium surgery,the corneal astigmatism,total wavefront aberration,high-order aberration,coma aberration and trilobal aberration of 40 eyes of 34 patients with pterygium were decreased compared with those before surgery,and the differences were statistically significant(P<0.001).Conclusion:The length,width,thickness and area of pterygium invading cornea are related to preoperative wavefront aberrations.The length,width and area of pterygium invading cornea may be more important.The wavefront aberrations were also related to the length,width and area of pterygium.Therefore,from the visual quality after pterygium surgery,the length,width and area of pterygium invading the cornea can be used as an objective index for the timing of pterygium surgery. |
