| Objective:The levels of IL-5,IL-4,IL-2 and INF-γ in serum of Severe mycoplasma pneumoniae pneumonia and common mycoplasma pneumoniae pneumonia were analyzed to explore the expression and significance of IL-5,IL-4,IL-2 and INF-γ in serum of different degrees of mycoplasma pneumoniae pneumonia,so as to find a more objective and operable indicator for predicting severe mycoplasma pneumoniae pneumonia.Methods:Selected in July 2019 to January 2020 in the second affiliated hospital of kunming medical university diagnosis of children with mycoplasma pneumoniae pneumonia with complete medical records as the research object,according to the illness into the severe group and ordinary group,normal group with 58 cases,21 cases of severe group,and randomly selected in the same period children medical care clinic children 36 cases as control group.Fast-fasting blood was collected for Mycoplasma pneumoniae titer test on the morning after admission.After centrifugation,the upper serum was collected and stored in the refrigerator,and then centralized for examination.IL-5,IL-4,IL-2 and INF-γ were determined by double antibody sandwich method.SPSS26.0 statistical software was used to collate and analyze all the data.Numeration data were expressed as cases and percentages,and the column and column chi-square test was used.First,normality test and homogeneity test of variance were conducted on the measurement data.The mean±standard deviation was used as the representation,and the comparison between two independent samples was performed by the single sample t test.IL4,IL5,IL2 and INF-y were non-normal distribution,represented by median M(p25,p75).Nonparametric rank sum test was performed for comparison between groups.ROC curves were drawn for all indicators,and the logarithm of the negative correlation indicators was taken.For all results,P<0.05 was considered statistically significant.Results:1.Gender and age comparison of the severe group,normal group and control group:P value was 0.691 for gender comparison of the severe group,normal group and control group,and the difference was not statistically significant(P>0.05);The age was(3.59±3.68)years old,(3.58±2.52)years old,and(3.52±2.11)years old,respectively.There was no statistical significance in pair comparison(P>0.05).2.Comparison of serum IL-5,IL-4,IL-2 and INF-y levels in the severe group,normal group and control group:(1)Comparison of IL-5 levels:76.54(64.21,95.05)pg/ml,61.79(53.21,75.02)pg/ml and 33.32(25.61,45.48)pg/ml in the severe group,normal group and control group,respectively,P<0.05.(2)Comparison of IL-4 levels:the levels of the severe group,the normal group and the control group were 33.57(29.54,40.18)pg/ml,30.17(25.65,35.36)pg/ml and 19.71(12.91,22.93)pg/ml,respectively,P<0.05.(3)Comparison of IL-2 levels:4.76(4.28,5.42)pg/ml,6.68(5.79,7.47)pg/ml,8.35(7.31,8.99)pg/ml in the severe group,the normal group and the control group,respectively,P<0.05.(4)Comparison of INF-y levels:513.98(431.89,616.27)pg/ml,478.55(381.79,557.27)pg/ml and 293.67(184.52,385.94)pg/ml in the severe group,normal group and control group,respectively(P<0.05).3.The area under the logarithmic ROC curve(AUC)of IL-2 was the largest,the AUC value was 0.923,the sensitivity was 81.0%,and the specificity was 87.3%.IL-5 and IL-4 had the highest specificity,with specificity of 98.3%,but poor sensitivity of 47.6%and 33.0%,respectively.AUC values were 0.744 and 0.678,respectively.Among the four cytokines,the one with poor sensitivity and specificity was INF-γ,with a sensitivity of 47.6%,a specificity of 87.9%,and the minimum AUC value of 0.640,P<0.05.Conclusions:1.The levels of IL-5,IL-4 and IL-2 in the serum of the children with Mycoplasma pneumoniae pneumonia were significantly abnormal compared with the healthy controls(IL-5 and IL-4 were increased and IL-2 was decreased),suggesting that the disorder of Th1 and Th2 cells represented by them was involved in the occurrence and development of Mycoplasma pneumoniae pneumonia.2.Serum IL-5 and IL-4 in the children with Severe mycoplasma pneumoniae pneumonia were significantly higher than those in the normal group,and IL-2 was significantly lower than that in the normal group.The area under the ROC curve of the logarithm of IL-2 was the largest,suggesting that the decrease of IL-2 could be used as an important indicator for the early warning of severe cases. |
