ObjectiveTo investigate the comparative study on the predictive value of CHA2DS2-VASc score and neutrophil/lymphocyte ratio for contrast-induced nephropathy in patients after elective percutaneous coronary intervention.MethodsA total of 300 patients undergoing elective percutaneous coronary intervention in Yiyang Central Hospital were enrolled in this study.CIN was defined as an increase in serum creatinine more than 44.2μmol/L or25%from baseline within 48 hours after contrast exposure.Baseline data and laboratory data were compared among the CIN group and the Non-CIN group.Multivariate logistic regression and Receiver operator characteristics(ROC)curves were used to assess the predictive value of CHA2DS2-VASc Score and neutrophil/lymphocyte ratio for contrast-induced nephropathy.ResultsThere were 208 male patients and 92 female patients enrolled into this study with an average age of 63.1±10.9.CIN occurred in 33(11%)patients.The proportions of female gender,age≥75 years,height,weight,emergency operation、hypertension、diabetes、congestive heart failure and previous stroke/TIA/thromboembolism history in the CIN group were significant higher than the Non-CIN group(P<0.05).Multivariate logistic regression analysis showed that CHA2DS2-VASc score(OR=8.229,95%CI:2.083~32.508,P=0.003)、NLR(OR=1.322,95%CI:1.145~1.526,P=0.000)was an independent predictor of CIN.ROC analysis revealed that the optimal cutoff value of CHA2DS2-VASc score to predict the onset of CIN was 2.5,while the sensitivity was 97.06%and the specificity was 55.26%.The AUC was 0.817(95%CI:0.768-0.859).ROC analysis revealed that the optimal cutoff value of NLR to predict the onset of CIN was 4.43,while the sensitivity was 90.9%and the specificity was68.5%.The AUC was 0.816(95%CI:0.768-0.858).Med Calc software was used for pare comparison under ROC curve Z:0.014,(95%CI:-0.086-0.087),P=0.989.The results showed that there was no difference between the CHA2DS2-VASc score and NLR for the predictive value of CINConclusionsCHA2DS2-VASc score and NLR could predict the CIN in patients after PCI.There is no difference between the CHA2DS2-VASc score and NLR in predicting CIN.
