Objectives:To understand the current status of communication between breast cancer patients and their underage children and the content of their needs for information about their mother’s cancer conditions.Provide a basis for constructing a disease communication program between breast cancer patients and their underage children.Based on the family system theory.Develop intervention programs for communication between breast cancer patients and their underage children.Explore the effectiveness of intervention programs.Methods:(1)Part 1:Qualitative research on the communication between breast cancer patients and their underage children.After consulting relevant domestic and foreign literature,combining the research purpose and my clinical practice,the interview outline was formed through the guidance of the instructor and pre-interview.According to the inclusion and exclusion criteria,select 11 breast cancer patients as interview subjects,and sort out the main problems in the communication between breast cancer patients and their underage children,which were the conditions of breast cancer patients and underage children.Provide a basis for the construction of communication intervention programs.(2)Part 2:The construction of an intervention plan for communication between breast cancer patients and their underage children based on the family system theory.Through literature search,collect domestic and foreign literature on interventions for children of cancer families,and conduct an overall analysis of the literature.The research team constructed the first draft of the intervention plan based on the previous interview questions,combined with the literature search results,clinical experience and scientific research experience.Sort out and analyze expert consultation and feedback,and revise and adjust the intervention plan.Then form the final draft of the intervention plan.(3’)Part 3:Application of Intervention Program for Disease Communication between Breast Cancer Patients and Minor Children.From July 2020 to December 2020,according to the order of admission,they were divided into experimental group and control group randomly.The control group received routine nursing care,and the experimental group implemented breast cancer patients and minor children’s condition communication intervention on the basis of routine nursing care.The program mainly included four themes of breast cancer knowledge education,emotional management,disease notification strategies,and mother-child relationship strategies.Three interventions were performed on the first day after admission,before discharge,and one week after discharge.Before the intervention,1 month after the intervention,and 3 months after the intervention,the Child Depression Scale,State Anxiety Scale and Family Function Scale were used to evaluate the anxiety,depression and family function of breast cancer patients and their children.Results:(1):the qualitative research results were summarized into 4 themes:illness communication,emotional experience,positive power gained,and protection of children.(2)Seven articles were included.The literature was summarized and sorted,and the research design,intervention objects,sample size,intervention content,intervention form,intervention duration,measurement tools,and evaluation time were preliminarily determined after discussion by the research group.Eight experts gave their opinions.(3)Application of intervention programs.A total of 54 patients were enrolled in this study,the effective sample size was 50,and the loss to follow-up rate was 7.40%.Depressed mood in children.Before the intervention,the score of the control group was 26.36±2.69,and the score of the experimental group was 26.48±2.95.The results of repeated measures analysis of variance showed that the difference in effect between the groups was statistically significant(F=5.635,P<0.05);the difference in depression scores between the two groups at different time points was statistically significant(F=39.108,P<0.01));There was an interaction effect between time and grouping(F=6.037,P<0.01).The patient’s anxiety.Before the intervention,the score of the control group was 43.84± 7.36,and the score of the experimental group was 42.92±4.75.The results of repeated measures analysis of variance showed that there was a statistically significant difference in effect between the two groups(F=4.672,P<0.05);there was no significant difference in depression scores between the two groups at different time points(F=2.583,P>0.05));There was an interaction effect between time and grouping(F=3.515,P<0.05).Family function.Before the intervention,the total family function score of the control group was 133.12±19.42,and the score of the experimental group was 131.64± 12.51.The t-test results of two independent samples showed that the differences were statistically significant in the four dimensions of communication,role,emotional response,and emotional intervention 1 month after intervention and 3 months after intervention.There was no statistically significant difference between the two groups in the three dimensions of problem solving,behavior control,and total function.Conclusions:(1)Based on the family system theory,the intervention program of breast cancer patients and their children’s condition communication is beneficial to alleviate the anxiety and depression of breast cancer patients and their children,and promote the family function of patients.(2)The intervention plan can provide a basis for the formulation of an intervention plan for clinical patients and minor children’s condition communication. |
