| Objectives:There are three objectives of this paper.They are to explore the relationship between serum lipid level,uric acid and colorectal adenomatous polyps,to explore the correlation between blood lipid level and pathological characteristics and development characteristics of colorectal adenomatous polyps,and investigate the related factors of colorectal adenomatous polyps in order to control the occurrence and development of colorectal adenomatous polyps.Methods:In this paper,we have collected 167 patients with colorectal adenomatous polyps from the Affiliated Hospital of the second affiliated Hospital of Kunming Medical University to analyze retrospectively,from January 2016 to December 2020.Among them,95 patients with colorectal adenomatous polyps were adenoma group,72 patients with normal colonoscopy were control group.The differences of clinical general data(including age,nationality,gender,smoking history,drinking history,BMI,hypertension history,type 2 diabetes history),blood cell analysis(WBC count,NEUT count,HGB concentration,PLT count)and biochemical indicators(UA,TC,TG,HDL-C,LDL-C,FBG)between the two groups were compared.According to the characteristics and development direction of colorectal adenomatous polyps,the differences of BMI and biochemical indexes(uric acid,triglyceride)were analyzed.As for these two groups,Chi square test was used to compare the count data;independent sample t-test was used to compare the normal distribution of measurement data;Mann Whitney U test was used to compare the non normal distribution.Logistic regression analysis was used to explore the risk factors of colorectal adenomatous polyps.The data were analyzed and processed by spss26.0 software,and the statistical value was statistically significant(P<0.05).Results:1.The male[66(69.5%)VS 25(34.7%)],age[62(55,71)VS 56(49,66)],smoking history[51(53.7%)VS 13(18.1%)],drinking history[42(44.2%)VS 14(19.4%)],BMI(24.05±3.54 VS 22.00±3.16),WBC count(6.13±1.93 VS 5.52±1.91),NEUT count(3.70±1.74 VS 3.08±1.44),HGB concentration(146.97±18.37 VS 133.40±20.80),PLT count(209.85±65.15 VS 241.32±85.23),UA(374.80±90.16 VS 326.40±82.08),TG[1.42(1.15,2.09)VS 1.26(0.87,1.69)],FBG[5.17(4,84,5.63)VS 4.93(4.60,5.32)]levels of colorectal adenomatous polyps group were higher than those of control group,and the differences were statistically significant(P<0.05).There were no significant differences in hypertension history[31(32.6%)VS 18(25.0%)],type 2 diabetes history[12(12.6%)VS 5(6.9%)],TC[4.40(3.78,5.30)VS 4.52(3.77,5.27)],HDL-C[1.19(1.03,1.38)VS 1.23(1.03,1.54)],LDL-C(2.80±0.78 VS 2.81 ±0.93)between adenomatous polyps group and control group(P>0.05).2.Multivariate logistic regression analysis was used to analyze the risk factors of colorectal adenomatous polyps.The results were as follows:age(OR=1.079,95%CI:1.032~1.127),smoking history(OR=0.101,95%CI:0.023~0.439),BMI(OR=1.176,95%CI:1.021~1.354),WBC count(OR=0.403,95%CI:0.211~0.771),NEUT count(OR=3.496,95%CI:1.592~7.679),HGB concentration(OR=1.031,95%CI:1.007~1.056)were independent risk factors of colorectal adenomatous polyps(P<0.05).3.Comparison of BMI,UA and TG levels according to the pathological characteristics and development direction of adenomatous polyps.3.1 The proportion of overweight,obesity(56.30%VS 22.06%)and abnormal TG(54.70%VS 9.70%)were higher in villous adenoma group than that in non-villous adenoma group(P<0.05);3.2 The proportion of abnormal UA(33.90%VS 24.20%)in multiple colorectal adenomas group was higher than that in single colorectal adenoma group(P<0.05);3.3 compared with adenoma diameter<0.5cm,colorectal adenoma diameter≥0.5cm had higher proportion of overweight,obesity(64.3%VS 30.23%)and abnormal TG(66.70%VS 18.90%)(P<0.05);3.4 The proportion of overweight,obesity(60.70%VS 17.60%)and abnormal TG(57.40%VS 8.80%)in adenoma with intraepithelial neoplasia group was higher than that in adenoma without intraepithelial neoplasia group(P<0.05).Conclusions:The male,age,Han nationality,smoking history,drinking history,BMI,WBC count,NEUT count,HGB concentration,PLT count,UA,TG and FBG levels were the influencing factors of colorectal adenomatous polyps.Multivariate logistic regression analysis showed that age,smoking history,BMI,WBC count,neut count and HGB concentration were the independent risk factors of colorectal adenomatous polyps.2.Overweight,obesity and hypertriglyceridemia have higher risk of high-risk adenoma.Hyperuricemia increased the number of CAP. |
