Clinical Study On The Treatment Of Blood Stasis Adenomyosis Dysmenorrhea Based On The Theory Of "Collateral Disease"

Objective: Based on the theory of collateral disease,this study explored the pathogenesis of adenomyosis,the patients with dysmenorrhea due to adenomyosis of blood stasis type were selected as the research objects,objective to analyze the clinical efficacy of Tongluo Huayu Tiaochong decoction,which was prepared by our tutor,in order to provide new ideas and methods for clinical treatment.Methods: A total of 64 patients with dysmenorrhea due to blood stasis type adenomyosis were collected from the gynecological clinic of Affiliated Hospital of Shandong University of Traditional Chinese Medicine from April 2018 to November2020,they were randomly divided into observation group(Tongluo Huayu Tiaochong Decoction)and control group(Guizhi Fuling Capsule),to observe the effect of improving the degree of pain,dysmenorrhea symptoms,blood stasis symptoms and TCM syndromes,and to make statistical analysis.Results: In the observation group,there was 1 case of exfoliation,1 case of elimination,and 2 cases of exfoliation in the control group,with a total of 60 cases completed.Before treatment,there were no significant differences in age,duration of disease,times of pregnancy,times of delivery,times of abortion,gynecological examination,uterine volume,degree of dysmenorrhea pain,dysmenorrhea symptoms,blood stasis symptoms and TCM syndromes between the two groups(P>0.05),it is comparable.In comparison between the two groups before and after treatment,there was no significant difference in uterine volume(P>0.05),but the differences in gynecological examination score,dysmenorrhea pain score,dysmenorrhea symptom score and blood stasis symptom score were all statistically significant(P<0.01);In terms of TCM syndromes,the difference in the scores and total scores of TCM syndromes in the observation group before and after treatment was statistically significant(P<0.01),the control group was compared before and after treatment,there were significant differences in menstrual color,menstrual volume,menstrual period and pulse syndrome score(P<0.05),and the differences in the scores and total scores of other TCM syndromes were statistically significant(P<0.01).Comparing the observation group with the control group,the differences in gynecological examination scores,dysmenorrhea symptom scores,usual waist and abdomen pain syndrome scores,and TCM syndrome total scores after treatment were statistically significant(P<0.01),the differences in pain degree scores of dysmenorrhea,blood stasis symptom scores and other TCM syndrome scores were statistically significant(P<0.05);After treatment,the two groups of patients had statistically differences in dysmenorrhea efficacy and blood stasis efficacy(P<0.05),the difference in curative effect of TCM syndromes was statistically significant(P<0.01).Conclusion: The pain meridian of adenomyosis conforms to the theory of "chronic disease enters the collaterals" and "chronic pain enters the collaterals",the essence of collateral disease is collateral obstruction,which belongs to "blood stasis",which is consistent with the pathogenesis of dysmenorrhea in adenomyosis.Through clinical research,it is found that Tongluo Huayu Tiaochong Decoction based on collateral disease theory can effectively improve the score of gynecological examination and the degree of dysmenorrhea,and effectively reduce the symptoms of dysmenorrhea,blood stasis and TCM syndrome score,it has significant effect on dysmenorrhea of blood stasis type adenomyosis,therefore,it is worthy of further clinical research and promotion.
