Objective: To explore the group cognitive behavior therapy for chronic insomnia in the elderly pension agency application effect,cognitive behavioral therapy intervention by groups,improve the pension agency the sleep quality of old people,reduce anxiety,depression and other negative emotions,for group cognitive behavior therapy in pension institutions provide clinical basis for the application in the elderly.Methods: Through the literature,this study found that the problem,the questionnaire,such as process,clear objective,and build the intervention plan the intervention scheme of the present study was to group cognitive behavior therapy,including cognitive therapy,sleep hygiene education,stimulus control therapy,and relaxation therapy,relaxation therapy in this study for group cognitive behavior therapy for innovation,by using self head acupuncture point massage to relax,invited expert evaluation,and organize the team discussion,after preliminary experiments,perfect and determine the intervention plan.The research team of this study was composed of seven researchers,including one psychotherapist,one traditional Chinese medicine physician,one graduate student,two nursing counselors,and two nursing assistants.The graduate student was the main investigator to implement the whole intervention program.A total of 86 elderly people with chronic insomnia who lived in Chongyangcheng Apartment for the Aged in Nanning City,Guangxi Zhuang Autonomous Region and who met the evaluation criteria of inclusion and exclusion were selected as the research objects by convenience sampling method.According to the computer random number table method,they were randomly divided into observation group and control group,with 43 cases in each group.The control group was given routine sleep health education,once every 4 weeks,a total of 3 times,a period of 12 weeks;Observation group was given conventional sleep health education on the basis of combined with group cognitive behavioral therapy intervention,group cognitive behavioral therapy intervention once a week,a total of 12 times,group cognitive behavioral therapy and conventional sleep health education at the same time,a period of 12 weeks.The whole research program is carried out by graduate students.The time,frequency,process and content of the intervention are consistent to ensure the consistency of the research.Before the intervention,the subjects and their family members agreed and signed the informed consent,and the researchers conducted a questionnaire survey on the subjects with the General Situation questionnaire,Pittsburgh Sleep Quality Index(PSQI),Insomnia Severity Index(ISI),Hamilton Depression Scale(HAMD),and Hamilton Anxiety Scale(HAMA).At the end of the12-week intervention,the researchers assessed the subjects using the Pittsburgh Sleep Quality Index(PSQI),Insomnia Severity Index(ISI),Hamilton Depression Scale(HAMD),and Hamilton Anxiety Scale(HAMA).The questionnaires were collected on the spot and the scores of all the scales were recorded.The data were double-checked.By SPSS 26.0 software for data processing and analysis,all the data for normality and f test,found in this study data are not accord with normal distribution,so this study is compared between two groups between the object of study group and the use of two independent samples nonparametric test,comparison in the group choose two paired samples nonparametric tests,the results P < 0.05 difference was statistically significant.Results: 1.Comparison between groups: after intervention,sleep time,sleep efficiency and PSQI total score in the observation group were significantly lower than those in the control group,with statistical significance(P < 0.05);The difficulty in falling asleep,difficulty in maintaining sleep and total score of ISI in the observation group were significantly lower than those in the control group,with statistical significance(P < 0.05).The total score of HAMD in the observation group was significantly lower than that in the control group,with statistical significance(P < 0.05).The scores of stress,depression and HAMA in the observation group were significantly lower than those in the control group,with statistical significance(P < 0.05).2.Compare within groups: after intervention,the total score of PSQI in the observation group was significantly lower than that before intervention,with statistical significance(P < 0.05);The total ISI score of the observation group was significantly lower than that before intervention,with statistical significance(P < 0.05).The total score of HAMD in the observation group was significantly lower than that before intervention,with statistical significance(P < 0.05).The total score of HAMA in the observation group was significantly lower than that before intervention,with statistical significance(P < 0.05).Conclusions: 1.The group cognitive behavioral therapy with self-head acupoint massage for relaxation has a good effect,which improves the sleep condition of the elderly suffering from chronic insomnia in nursing institutions,improves the sleep quality,and relieves negative emotions such as anxiety and depression.2.Group cognitive behavioral therapy has a good effect on the elderly suffering from chronic insomnia in the pension service institutions.The organization and management of the pension service institutions are good,the human resources are sufficient,the research objects are stable,the elderly are concentrated and the residence time is long,which improves the efficiency,feasibility of the research and the compliance of the research objects,and it is worthy of promotion in the pension service institutions. |
