Objective:To investigate the effect of Xingju Mixture on the clinical symptoms and inflammatory indexes of senile acute cough,and to explore the possible mechanism of its therapeutic effects through network pharmacology.To provide evidence for treatment of senile acute cough with Chinese medicine.Methods:(1)The 60 elderly patients with acute cough as the main symptoms were hospitalized in the Department of Geriatrics,Nanjing Traditional Chinese Medicine Hospital,from January 2019 to September 2020,and randomly divided into 30 cases in the experimental group and 30 cases in the control group.The control group were treated with conventional anti-infection and phlegm treatment in the department of geriatrics,and the experimental group were treated with Xingju mixture orally.To compare the changes in clinical symptom scores on the 0-3-5-7th day of treatment,and the changes of inflammatory indexes on the 0-3-7th day of treatment,and to evaluate the safety of 2 groups.(2)The TCMSP was used to retrieve the active chemical components and targets of the 4 traditional Chinese medicines in Xingju mixture.The disease targets of senile acute cough were searched through the GeneCards and OMIM.The two were mapped to each other to screen the common targets between drugs and disease,and the interaction network diagram of the common targets was constructed by String.Cytoscape3.7.0 was applied to construct the mechanism network model of Xingju Mixture in the treatment of senile acute cough.The GO and KEGG enrichment analysis of targets were based on DAVID,searching the signal pathways that may play an important role in the treatment process.Results:(1)In the process of clinical observation,1 case of the control group was fell out,all patients in the experimental group completed the observation normally.Finally,59 cases were included,30 cases belonged to the experimental group and 29 cases belonged to the control group.The total effective rate of the experimental group was 96.7%,which was significantly higher than that of the control group(79.3%)(P<0.05).After the treatment of 7 days,the two groups’ cough symptom score,concomitant symptom score,and overall symptom score were significantly reduced,and the cough symptom score and overall symptom score of the experimental group decreased more than the control group(P<0.05).There was no significant difference in the decrease of the concomitant symptom score between two groups(P>0.05).In terms of laboratory indicators,the level of CRP in experimental group was significantly lower than that in control group(P<0.05),but there was no significant difference in the level of WBC and N%between two groups(P>0.05).In terms of safety,there were no adverse events such as liver and kidney damage in the two groups.(2)According to network pharmacology research,there are 103 active chemical components in Xingju mixture,such as quercetin,corresponding to 255 drug targets.131 targets are obtained by mapping with 995 targets of senile acute cough,including PTGS2,ESR1,CALM1,etc.The enrichment analysis mainly involves TNF signaling pathway,HIF-1 Signal pathway,etc.Conclusion:Xingju mixture combined with conventional western medicine treatment can further enhance the clinical effectiveness of senile acute cough,improve the clinical symptoms,and control the level of CRP safely and effectively.The Network pharmacology research has initially confirmed the possible action mechanism of Xingju mixture in the treatment of senile acute cough with multiple targets and multiple pathways,which provides more ideas and directions for follow-up research.
