Objective:Data of this study is based on the fourth to the sixth Health Service Survey in Jiangsu Province as well as the sixth Health Services Survey research,through the analysis of chronic diseases among residents in Jiangsu Province in nearly a decade of the historical change trend,and on the skill survey data obtained in 2018 research,detailed analysis of factors influencing the urban and rural residents in Jiangsu Province the complexity of the chronic diseases,respectively,and discusses the geographical factors influencing the urban and rural residents in jiangsu province differences between chronic diseases and endowment of factors.In order to reduce the prevalence of chronic diseases,narrow the difference between urban and rural areas of chronic diseases,promote the health equality between urban and rural areas,formulate reasonable health policies,and promote the development of health level in Jiangsu Province to provide data support.Methods:Firstly,review and sort out the related research on chronic diseases at home and abroad through literature research,and then based on the 2018 Health Service Survey data in Jiangsu Province,9758 people aged 15 and above were selected as the research subjects,and the sociodemographic characteristics,behavioral and lifestyle characteristics,and socioeconomic characteristics of the included subjects were collected,Using the health ecological model as the theoretical basis,the individual traits,behavioral characteristics,interpersonal network,social and economic status were analyzed by univariate analysis,and the Logistic multi-factor regression model was constructed to analyze the main factors affecting the chronic disease status of urban residents and rural residents.On this basis,the Shapley value decomposition model was built to decompose the regression model of significant variables to study the contribution rates of different variables to the chronic diseases of urban and rural residents.Further,by using the nonlinear Oaxaca-Binder decomposition method,the differences of chronic diseases among urban and rural residents were decomposed,and relevant policy suggestions were put forward.Results:The prevalence rate of chronic diseases of residents in Jiangsu Province is increasing year by year and the rate is fast,the urban area is higher than the rural area,while the rural area is growing faster than the urban area,and the gap between urban and rural areas is narrowing gradually.Rural females are higher than males;Sick population mainly concentrated in the elderly population;The prevalence of chronic diseases is highest in low-income groups;Hypertension and diabetes are the top two chronic diseases.Multivariate Logistic regression analysis showed that age,BMI,physical examination,brushing teeth,education level and employment status in urban areas had a significant impact on the prevalence of chronic diseases,while age,BMI,physical examination,exercise,brushing teeth and employment status in rural areas had a significant impact on the prevalence of chronic diseases.The decomposition results of Shapley’s value showed that the factors causing the risk of chronic diseases in urban areas were age,education level,employment status,tooth brushing,physical examination and BMI,while the factors causing the risk of chronic diseases in rural areas were age,employment status,BMI,tooth brushing,physical examination and exercise.The results of nonlinear Oaxaca-Binder decomposition showed that the main factors affecting the prevalence of chronic diseases between urban and rural areas were urban and rural regional factors,education level,brushing teeth,age,physical examination,exercise,per capita annual income,employment status,BMI,family size,and medical insurance participation.Conclusion:In the current society with rapid social and economic development,the prevalence of chronic diseases has been high,and there is a very obvious difference between urban and rural areas.It is suggested to further strengthen the tertiary prevention of chronic diseases for both urban and rural residents,especially the importance of prevention of causes,strengthen health education and publicity to help urban and rural residents develop healthy living habits,but also pay attention to the elderly group,by improving the long-term security system of chronic diseases of the elderly to reduce the prevalence of chronic diseases;The reform of the household registration system should also be deepened in urban and rural areas,and more targeted chronic disease prevention and control measures should be taken according to local conditions,such as increasing investment in education in rural areas.Through comprehensive exploration of effective prevention and control of chronic disease prevalence,improve the health status of residents,to escort "Healthy Jiangsu" and "Healthy China 2030".
