| Objective:The purpose of this study was to observe the effects of electroacupuncture and routine treatments on the efficacy of Lumbar disc herniation(LDH)and the effects of electroacupuncture on the efficacy of different LDH fat infiltration of multifidus muscles.To evaluate the Oswestry dysfunction index score(ODI),Medical Outcomes Survey Short Form-36(SF-36)before and after treatment,and measurement the cross-sectional area(CSA)and Fatty infiltration rate of multifidus muscle(FI)before and after treatment,so as to find an effective evaluation method for patients with lumbar disc herniation,and to explore the mechanism of electroacupuncture treatment.Method:168 patients with LDH were recruited,and the prominent segment was L4/5(and/or)L5/S1.1.108 patients were used Image J software and MR T2-weighted images(T2WI)to measure the FI of the lower marginal plane of the L4 vertebral body.According to the results,they were divided into group A(FI<10%)and group B(10%).≤FI≤20%),group C(FI>20%),36 cases in each group.All patients were given electroacupuncture treatment,once every other day,10 times as a course of treatment,a total of 1 course of treatment.The three groups were evaluated ODI and SF-36 before and after treatment.Using SPSS23.0 statistical software,the three groups of ODI and SF-36 were used for statistical analysis within and between groups before and after treatment.2.According to the random number generation method,60 patients were divided into control group 30 cases and experimental group 30 cases.Both groups of patients received health education and symptomatic drug treatment.The experimental group was treated with electroacupuncture,once every other day,10 times as a course of treatment,a total of 2 courses.Both groups were evaluated for ODI and SF-36 before and after treatment。And before and after treatment Image J software and MR T2-weighted images(T2WI)were used to measure the CSA and FI of the bilateral multifidus muscle at the lower edge plane of the L4 and L5 vertebral bodies,and take Mean.Using SPSS23.0 statistical software,the two groups of ODI,SF-36,CSA,and FI were used for statistical analysis within and between groups before and after treatment.Result:1.There was no statistical difference in general data such as age,gender,course of disease,and BMI among the three groups before treatment(P>0.05).There were no statistically significant differences in the physiological dimensions(PCS),psychological dimensions(MCS)and ODI of SF-36 in the three groups before treatment(P>0.05).After treatment,the scores of ODI,PCS,and MCS in the three groups were improved compared with those before intervention(P<0.05);among them,in ODI,PCS,the differences between the three groups after treatment were statistically significant(P<0.05),the lower the FI value,The better the effect;in MCS,there is no statistically significant difference between the three groups after treatment,but there is a certain trend(P=0.078).2.Before treatment,there was no statistical difference in general data such as age,gender,course of disease,and BMI between the two groups(P>0.05).Before treatment,there was no statistical difference in PCS,MCS,ODI,FI,CSA between the two groups(P>0.05).PCS,MCS,and ODI were statistically different in the control group before and after treatment(P<0.05);there were no statistical differences in FI and CSA(P>0.05).PCS,MCS,ODI and FI of the experimental group were statistically different before and after treatment(P<0.05);CSA was not statistically different(P>0.05).After treatment,PCS,ODI and FI were statistically different between the two groups(P<0.05);MCS and CSA were not statistically different(P>0.05).Conclusion:1.Routine treatment for patients with lumbar disc herniation can significantly improve pain,physical and psychological dysfunction,but the shape and nature of the multifidus muscle cannot be significantly improved.2.The electroacupuncture group can also significantly improve pain,physical and psychological dysfunction,and can improve the fat infiltration rate of the multifidus muscle,thereby rebuilding the stability of the spine.It is worth noting that it is significantly better than conventional treatment for improving pain and physical disorders,but it has no obvious advantages for improving psychological dysfunction.3.The fat infiltration rate of multifidus muscle may be one of the reasons that affect the pain and function improvement.Clinically,attention should be paid to the changes in muscle morphology and structure. |
