The Study On The Clinical Observation Of Buqi Xiaodian Decoction In The Treatment Of Immunologic Thrombocytopenia And The Intervention On The Expression Of TLR2

Objective: To observe the clinical effect of Buqi Xiaodian decoction combined with glucocorticoid in treating Immune Thrombocytopenia(ITP)of deficiency of Qi,to explore the effects of Toll-like receptor 2(TLR2)mRNA expression,Interleukin-6(IL-6)and Tumor necrosis factor(Tumor necrosis factor-alpha(TNF-α)levels in ITP patients,and to provide a new idea for treating ITP by integrated traditional Chinese and Western medicine.Methods: 40 patients with ITP were randomly divided into 2 groups:Treatment Group(20 cases)and control group(20 cases).The control group was treated with prednisone glucocorticoid and the treatment group was treated with Qibuxiaodian decoction on the basis of the control group,the expression of TLR2 mRNA,serum TNF-α and IL-6 in peripheral blood mononuclear cells of the treatment group and control group were recorded before and after treatment,at the same time,the expression of TLR2 mRNA in peripheral blood mononuclear cells,the level of serum tnf-α and IL-6in blank group were detected.Result: 1.After treatment,the total effective rate of TCM syndrome in the treatment group(85%)was higher than that in the control group(30%)(p< 0.05).The TCM syndrome score of the two groups was better than that before treatment(p < 0.05),and the treatment group was better than the control group(p < 0.05).2.Compared with Western medicine,the total effective rate(95%)of the treatment group was higher than that of the control group(80%),the difference was statistically significant(p < 0.05).3.Platelet count was higher in treatment group and control group after treatment(p < 0.05)PLT count in treatment group was higher than that in control group(p < 0.05).4.Compared with the control group,the bleeding score of the two groups decreased(p < 0.05),and the bleeding score of the treatment group was lower than that of the control group,the difference was statistically significant(p < 0.05).5.After treatment,the quality of life of the two groups was improved(p < 0.05),and the treatment group was better than the control group,the difference was statistically significant(p < 0.05).6.After treatment,the incidence of dry mouth and insomnia in the treatment group was lower than that in the control group(p < 0.05),and the incidence of headache,excitement or depression in the two groups was lower(P > 0.05).7.The levels of TLR2 mRNA,serum IL-6 and tnf-α in ITP patients were higher than those in control group(p < 0.05).After treatment,the levels of TLR2 mRNA and serum IL-6 and tnf-α in ITP patients were lower than those in control group(p < 0.05),the level of TLR2 mRNA in the treatment group was lower than that in the control group(p < 0.05),and the levels of IL-6and tnf-α were similar in the two groups(P > 0.05).8.There was a negative correlation between PLT count and TLR2 mRNA expression,and a positive correlation between TLR2 mRNA expression and serum tnf-α and IL-6 levels.Conclusion: BQXD combined with glucocorticoid can improve the clinical symptoms of ITP patients with qi-deficiency type,reduce the adverse reaction of glucocorticoid and improve the quality of life,the expression of TLR2 mRNA in peripheral blood mononuclear cells and the levels of tnf-α and IL-6in serum were decreased,and the PLT count was increased in ITP patients.
